Question: Hello, I just got my arm extended at a grappling tournament.. I have a sharp pain in my shoulder, my bicep is bruised and sometimes having spasms in my shoulder and bicep when i move. also the connecting area between my bicep and shoulder is swollen/sticking out a little. had trouble rotating in the front and back. medic at the tournament said it could be a strained muscle or a dislocation that popped back in but its gotten better but still having pain moving such as turning the steering wheel or taking off a shirt. i fear some kind of tear.

I had gone through your question and understand your concerns ..

In my opinion i think it is a muscle sprain due to overusing and overstretching your shoulder joint , avoid heavy lifting , vigorous movement and wearing an arm sling is helpful .

For now painkillers like ibuprofen when needed , muscle relaxants , hot massages with topical gels , use an arm sling or arm elevation to decrease swelling and try to give it time to heal and avoid vigorous exercise and relax for the first 2 to 3 weeks .

As long as you could fully rotate your shoulders effectively even while having pain is fine . if pain persists i suggest MRI scan to exclude any torn muscle for proper evaluation and management .

My bruising was increased over night on my bicep. is this a concern?

Also, is there a reason why you would use heat over ice?

Detailed Answer:
Try to elevate it above your head a little bit and avoid a dropped hand sleeping postures and avoid sleeping on the injured side .
Try the topical gels three times per day after hot foments or massaging it gently with elevating it a little bit to prevent it from more inflammation and swelling , wearing an arm sling or slabbing it is helpful and avoid dropping it that prevents blood flow going back towards the heart thus causing the swelling .
usually icing it for the first 48 hours is very helpful with the swelling and prevents excessive swelling , after the first 2 days the hematoma or the blood clot or bruise needs hot massages to increase the blood flow and thus dissolving that hematoma or the clot by washing out the inflammatory mediators causing the swelling
