Suggest Treatment For Severe Pain In The Ear

Question: My daughter is 9 years old and each time she gets a cold she also complains of an ear ache. Previously just a month back her ear ache was terrible and it resulted in bleeding from the left ear. She had to be admitted in the hospital and had to be administered antibiotic injections to control her ear infection. Now again she has a cold and is complaining of slight ear pain in the right ear? I had ear drop Biocip (Ciprofloxacin) AT HOME so i administered 5 drops once.
I have attached the hospital report of about a month back.
My question is : 1. What needs to be administered to her right away for immediate relief for her ear pain? what is the long term solution?

Thank you for your query.

1. You can repeat a dose of medication in her discharge summary. Otrivin nasal drops, Allegra-M tablet once. You can avoid Forenza 325 at present. Steam inhalation will help. The ciprofloxacin ear drops are safe. An anti-inflammatory medication can be given after informing your doctor.

2. Rule out adenoid hypertrophy with an X-ray skull lateral view for adenoids.

3. Once she is better, get PTA ( Pure Tone Audiometry ) and I / A ( Impedance Audiometry / Tympanometry ) done.

4. Allergy can be tested.

5. Upload theses test results for discussing further treatment options.

Hi!
Good morning Doc.

Thank you for your advice. Today being Sunday , the OPD at the hospital is closed and hence i am not in a position to meet the doc until Monday ie Tomorrow.

My ques are:-

1.Hence, can you please suggest an anti inflammatory medicine? and the dosage?

2.As per your advice , i will administer Allegra M...... also needed to know for how many days? as per the last discharge report it was given for 10 days.

3. Can i use the otrinoz nasal drops and the Biocip( Ciprofloxacin) ear drops simultaneously or do i use either one?

1. Paracetamol should be fine as an anti-inflammatory and mild pain-killer.

2. Give the Allegra-M for 10-15 days.

3. You can use the drops simultaneously. Otrivin in the nose and Ciprofloxacin in the ear.

4. Review with your doctor on Monday and let me know the progress.

Dear Doc ,
Good evening!

Today rishona was shown to her ent doc at TMH , as per your advice an xray was done and adenoids was ruled out by the doc. She has not complained of any ear pain today.

I am enclosing a copy of the xray as well as the prescription given by the doc today.
The ear drop(ciprofloxacin) was stopped but the otrivin is continued. Also since she was taking out yellowish Flem/mucus i think an antibiotic was added ZOCEF - CV 250 (Cefuroxime and Potassium Clavuuanate) for 5 days.

Allegra -m was discontinued and allegra for pediatric - in syrup form was started.
Pls. find enclosed today's prescription and Xray.

Kindly advice.
