Question: I have a 17 year old daughter with serious pain that starts in her chest and radiates around through her back and shoulders. Family doctor referred us to Neurologist and pain manager, they prescribed more Valium and codeine. A family friend suggested an internist to explore the source of pain rather then Neurologist...seems to make more sense.

I have gone carefully through your query and understand your concerns. I think that your daughter needs to be evaluated by an internist and it would be advisable for you to visit an internist rather than a neurologist. Your daughter needs to be evaluated for:
* Gastritis. Taking omeprazole once daily before breakfast should help. Also, taking plenty of green leafy vegetables is important.
* Pancreatitis. Blood for lipase and amylase levels should help in confirmation.

Our DO from previous home in Illinois, (just moved a month ago) who had previously seen Grace for this condition reviewed her case again and current condition. She suggested that it may be Costochondritis, as the original pain began during her high school lacrosse game just over 4 years ago, she thought that a mild trauma during that game may have injured her rib cage and she continued to play thru the end of season possibly aggravating the injury. The current spasms or events are so painful that my daughter has a hard time getting a full breath. After 8 days of this we had seen a decrease in spasms, about 10 to 12 hours, and diminishing severity but last night she had a longer more painful episode.

Thanks for writing back. Costochondritis is another valid possibility. Clinical examination is necessary in order to confirm. The pain can often be debilitating. You need to give her ibuprofen twice or thrice daily regularly for at least 3 days. Be sure to give it in full stomach. Hopefully that would provide relief.
