Suggest Treatment For Severe Migraine And Fatigue

Question: Hello Doctor,

I am having sever migraine, feeling fatigued from a few months now, also my migraine has started coming really frequently, It starts on the left side of my forehead, temple to be more accurate, goes down to my neck, and left arm. I feel really nauseous, dizzy, my speech gets slurred and I forget words, my vision gets blurry and I start feeling really sleepy but cannot sleep. It stays for a few hours, then come back again after a few hours.

I am feeling dizzy, light headedness, lost my appetite, I eat because I feel weak otherwise I just don't feel like eating at all. I am feeling a sinking feeling in my stomach. I feel constipated but also feel as if I need to use the restroom at the same time. My urine is dark yellow in color. I sometimes have blood in my stool. Also, from last few months there are brown spots on my torso, back and chest (please see the picture attached) and they keep increasing in number and some in size as well. They are dry but they do not itch. I am losing weight constantly from last year or so, my skin is turning pale and dark in color. I feel bloating in my stomach from past few weeks but there is no swelling. My stomach starts hurting a few minutes after I eat and the pain goes away after a few minutes or hours. My nails are a little brittle and scaly. The soles of my feet have turned really yellow during past few months.

Also, when my head hurts I feel as if my heart rate is dropping and I can barely stand. I used to have low blood pressure. I have prominent dark circles under my eyes. Sometimes, along with the headache I also feel a sharp pain in my chest especially on the left side when I inhale.

Welcome and thanks for your query.
You have a varient of migraine that also gives you neurological impairment during the attacks. Sometimes it is called hemiplegic migraine.
Too much Tylenol can damage your liver. It would be good to check your liver function tests to check whether it is the cause of your yellow discoloration and high colored urine.
There are simple medicines to reduce the frequency and severity of migraine attacks and using these will help you reduce the number of Tylenol and also help you have better quality life.
The skin lesion is Tinea versicolor and you can treat it successfully with a combination of oral and local medications.
The blood in the stools is probably due to injury to the rectum by passage of hard stools. You can also get yourself examined for piles if it persists after your stools are made soft and easy to pass using osmotic laxative like movicol.
