Suggest Treatment For Severe Lower Back Pain And Scoliosis

Question: My 16 year old son started to develop severe lower back pain about a week before he started having chest pains. We initially took him to the doctor for the back pain and the Dr. did an x-ray and said he has mild scoliosis. He has been doing physical therapy for several weeks which has done little to help the pain. He has seen a different Dr. for the chest pain, had an abnormal EKG (showing he had PAC). He had to wear a 48 hour heart monitor a week ago which we are still awaiting the results. His chest pains come and go, but are getting worse when he gets them. We took him back to the doctor yesterday who says she thinks his heart is fine, but ordered a chest x-ray which we are going to have done now, and then an echocardiogram next week. She thinks it might be a pulled chest muscle, which very well could be. However, I am wondering if maybe the lower back pain and the chest pain might be connected since they basically started around the same time I was just reading something about mild scoliosis that said you can also experience chest pain due to a bulging rib or twist of the spine. Could this be an issue we should be exploring as I feel we are spinning our wheels? Also, are there any other treatments for the lower back pain other than physical therapy, as he is in a ton of pain. Massage therapy? Chiropractic therapy?

Brief Answer:
other causes have to be excluded first

Detailed Answer:
Hello,

the combination of difficult to relieve symptoms after several weeks of treatment raises some suspicion for inflammatory causes like ankylosing spondylitis. There are some points to check from the history. For example pain which awakes him at night (particularly the 'second part' of the night) is in favor of inflammatory cause. Certain lab tests like the erythrocyte sedimentation rate (ESR) and HLA-B27 may provide additional clues.

If you could describe his symptoms in more detail and infomr me about the tests he's done so far it may help me to understand his situation better. Any other symptoms even if they seem unrelated will be helpful!

Kind Regards!
