Brief Answer:
Muscular pain most likely!

Detailed Answer:
Hi
Thanks for choosing health care magic for your query!
Echo and ECG are base line tests of cardiology. Sensitivity of 2d echo to rule out coronary artery disease is about 70%, combined together with tread mill test its 85%, and Stress thallium has a sensitivity of about 97% to rule out cardiac disease.

Your history is indicative of muscular origin of pain, most likely its because of inflammation or sprain of small muscle in chest. As the pain gets worse on lifting weights it again points toward muscular origin of pain. You can't lie on one side while sleeping; which again point towards involvement of particular group of muscles.

Higher levels of anxiety can also mimic symptoms of cardiac pain, Ask you cardiologist to keep you on low dose of benzodiazepenes like clonazepam; They are anti-anxiety medications and will control levels of your anxiety.

Though gastritis, oesophageal spasm and GERD (gastro esophageal reflux disease ) can also cause chest pain, very often they present with heart burn, postural variation of pain is also not seen in gastritis; so its highly unlikely in your case.

Paracetamol is a very weak anti inflammatory medicine, I would suggest you to take a combination of diclofenac with serratiopeptidase for 5 days, don't lift any weights or do heavy work. If after taking the medication pain still persists, go for stress thallium test. Stress thallium is highly sensitive for coronary artery disease.

Advice
Tread mill test
