Suggest Treatment For Severe Hair Loss

Question: Hi Doctor,

My age is 30. I am facing a severe Hair Fall and its happening for the past 10 years. It started in the year 2004-2005 when I had used Wella Hair Straightening Cream/Chemicals to straighten my hair.
Since then I have tried lots of hair treatments but the Hair fall has not stopped. I was having good density of hair with all black. But now for the past 1-2 years even the grey hair have started coming.

Whenever I had visited any Doctor I always told them how severe it is and how all it started but no doctor find using Straightening chemical as the main culprit. They says the 'alopecia' caused by heredity. But father and mother both have very healthy hair.
I tried Homeopathy, Aryurvedic and allopathic. Below are some treatments which I remember :
1) Folica Lotion (used in Year 2005) with some tablets as recommended by doctor.
2) Homeopathic treatment for 2-3 years from 3 different doctors for 4-5 months with each doctor(till 2008)
3) Richfeel treatment (2009-2010) for 5-6 months
4) Tugain lotion recommended by some doctor (2011) used for 4-5 months
5) Anaphase shampoo, androanagen tab, zorbfer tab, rhumafred tab, topsor lotion(.05) used for 3-4 months(2012)
6) Mesotherapy treatment(8 sittings) from DHI along with Mintop-5%(4 months) and Keraglo tablet for 1 month(2013)
7) Along with above treatment I tried using Ayurvedic oils like JRK Siddha's Kesh XXXXXXX Kesh King.

I stopped and tried other treatment because I never saw even 1% of improvement.

Please suggest what to do ?

I have reviewed the Images. You seem to have androgenetic alopecia.
The hair loss in Androgentic alopecia mainly involves the frontal and crown part of the scalp and is also associated with gradual recession of the anterior hair line.
There may also be hair loss and thinning from the angles of the anterior hair line so that the angles become wider and deeper (thus giving rise to a V shaped anterior hair line)
Family history is positive in most and but is not necessarily present in first degree family members like father, brother.
Positive family history in grandparents, aunts, uncles, nephews, nieces or half-siblings is also significant.
Only Minoxidil and finasteride have approval for use in this type of alopecia. Rest all are used as off-label (unapproved) treatments for hair loss with variable results.
Saw palmetto extract (which is an active ingredient in one of the tablets that you mention in point no. 5) has the same mechanism of action in androgenetic alopecia as finasteride and is also prescribed by dermatologist because it is supposed to be devoid of sexual side effects of finasteride.
While Minoxidil is a non-specific hair growth promoter, oral Finasteride and saw palmetto extract is specific for androgenetic alopecia in males.
Finasteride inhibits the enzyme responsible for converting testosterone to Dihydrotestosterone (DHT) in hair follicles. DHT is the main culprit in this type of hair loss.
Results are usually noticeable over a period of 6-12 months of regular minoxidil use and oral treatment.
The results are maximal at 1-2 years of combined treatment (Oral + topical) and then may be maintained with just Oral treatment.
Moreover, the goal with this therapy is to arrest or slow down further progression. New hair regrowth is an added advantage but response varies from individual to individual.
Therefore the aim is to start treatment as early as possible and to maintain the improvement with life long treatment.
A biotin supplement once daily would also be useful. Biotin supports hair growth though it is not specific for androgentic alopecia.
After going through the details of your query I can infer that you never took any treatment consistently for more than 4-5 months whereas ideally you should have never stopped or changed the treatment so frequently. You should have just continued with Minoxidil and Oral finasteride Or Saw palmetto extract..

You may also opt for a few sessions of platelet rich plasma therapy (PRP) in addition to oral and topical treatment.
The principle of this therapy is induction of new hair growth with the help of growth factors derived from one's own platelets.
Patients blood is withdrawn and centrifuged to concentrate platelets. The platelet rich plasma is separated and injected into the affected area to stimulate hair regrowth.
This therapy is a new approach to treatment of androgenetic alopecia and offers quite good results.

Hi.

PRP is based on induction of new hair growth by platelet derived growth factor. Patients own platelets are used.
Around 20-30 ml of blood is extracted and double centrifuged. Platelet rich plasma is separated and injected into the affected area of scalp.
3-4 sessions at a gap of 2-3 months may be required for good results.
Cost of treatment varies from doctor to doctor and would be anywhere from 5k to 10k per sessions.
You can ask for more details about PRP and availability from a dermatologist in your vicinity.

Along with PRP I would also suggest topical Minoxidil and Oral finasteride Or Saw Palmetto Extract. There is no need for any multivitamin.
Oral Finasteride and Saw palmetto are prescription drugs, therefore, I suggest you to talk to your dermatologist for the needful.

There are a lot of brands of Minoxidil 5% solution that are available over the counter.

Minoxidil should be applied on absolutely clean scalp.

Detailed Answer:
Hi

Minoxidil should be used on absolutely clean scalp. Since Minoxidil is available in an alcoholic base therefore it won't mix with Oil and Oil would form film impermeable to Minoxidil.
You may use Oil 2 hours before shampooing hair.

However, Minoxidil should be applied on absolutely clean scalp after Shampoo.
Moreover hair should be air dry and not wet when you apply Minoxidil because if you apply on wet scalp the concentration on minoxidil would get diluted.
