She is 23 years old (no psychiatric issues, finished 2 university degrees, resilient)
9 years of pain, began in lower back - all ortho and neuro issues ruled out
History of skin issues, frequent infections on skin, allergies, lactose intolerance, IBS - anti-nflammatories used, antidepressants, tylenol 3's to reduce pain (nothing ever worked)
January-March 2015 - severe lower abdomen pelvic pain - gyny workup ruled out any gyny issues and suggested GI. Bloating, back pain, anemia, mild bowel problems (constipation/diarhhea), celiac disease or IBD considered
April- present - gluten-free diet, no more bloating, nails growing, skin improved +++, anemia continuing, UTI, joint pain in knees, hips, fever, no menstrual issues (on birth control pill)
She can't stand the pain -affecting sleep and daily function

Brief Answer:
Seems IBS and/or IBD

Detailed Answer:

Thanks for the detailed information
Having ruled out most GI and gynaec issues, I feel she is suffering from IBS and or IBD

It is advisable to go for a stool examination, and if need be a colonoscopy

I would in the mean time advice her to take a good probiotic twice a day for four to six weeks

A tablet of Tramadol 75mg as and when need be should help her relieve pain

Gluten free diet does help reduce symptoms of bloating so she should continue the same

Hope this helps
Do write back in case of concern

she has an appoint on Nov 2, 2015 to see a Gastro specialist.
Family Dr not sure what to do, until then.
should she be seen sooner? will there be irreparable damage to intestines if IBD?
she is eating low FOD map foods by the way.
we are considering paying for her to see a specialist sooner than November.

Brief Answer:
It doesn't appear to be an emergency

Detailed Answer:

Thanks for the followup

It does not seem to be an emergency
I would suggest you take the medicines I have prescribed and keep in touch with me in case of need

Depends on what type of pathology is found on colonoscopy

She may continue with the same and gluten free diet

That is your personal decision to see a consultant prior to the appointment

Do not hesitate to write back in case of need
