Suggest Treatment For Severe Constipation After Child Birth

Question: I had normal delivery on sep 2015. From then I had problem with stitches which did not dissolve on its own. I delivered via episiotomy and forceps. Now it's 4 months plus and I had pus oozing from episiotomy scar tissue site. We took MRI scan and it said there is evidence of mixed signal intensity lesion noticed at episiotomy site on right side and extending in to right side ischio anal region with soft tissue pernial edema with small fistulous tract extending to postero vaginal vault with lower end noticed at 7'o clock position. Doctor then drained the abscess there and cleared the cavity. But it's been three days still am having the same pain. I had severe constipation till now and it's improving a little. Kindly help what can be done now and still there is pus oozing a little.

Your condition is worth being apprehensive about. There has been an wound infection which has progressed to create a fistulous tract. Constipation has complicated the problem. Right now you need to be patient. You need to take plenty of green leafy vegetables in diet and back it up with fiber supplements like psyllium husk or senna.

Keep the area clean and dry. Periodic dressing might be needed. Sitz bath might be helpful. Antibiotics might be required. Be in follow up with your doctor.

Kindly advice what is the specify treatment to be taken. Because I have already taken antibiotics and it don't work out for me. The pain is still there.

You need to see your surgeon every few days till it heals.

Detailed Answer:
You need to follow up with a surgeon. The first step of drainage of the abscess has been already done. Now the fistulous tract has to heal. Reinfection or stagnation of the infection has to be prevented and here the role of surgeon and periodic follow up with him comes in. Sitz bath will provide some relief. But unless the fistulous tract has healed, the pain will not go completely. Merely taking antibiotics would not eliminate the infection.
