Brief Answer:
Be patient , maintain physiotherapy and stretching exercises , B12 supplements will be helpful .
Hope you feel better soon i think you may have a sort of failed back syndrome.

Detailed Answer:
Hi i am Dr Ahmed Aly
thanks for using HealthcareMagic site ,
I had gone through your question and understand your concerns ..

I hope you feel better soon , it seems you had nerve compression to long ago , from your given history you seem to be a hypertensive patient , done physiotherapy , spinal straightening exercises and even yoga and pain medications .
At first you have to stabilize your cardiac conditions , maintain physical and straightening exercises , avoid heavy lifting , over weighing , bad sleeping or sitting postures and vigorous movements .
In my opinion you may have a failed back syndrome which means that your past neurological interventions like what you have mentioned before laminectomy or dissectomy have failed and thus not curing your nerve compression but increases your pain instead . such surgeries need a new correction surgery .
I do not think that further medication will help you especially with your daily hypertensive medications . physiotherapy sessions with a good chiropractor and stretching exercises will be more effective . you may add B12 supplements , muscle relaxants will help .
I personally recommend a new MRI scan on your spine and follow them up with a neurosurgeon for proper evaluation and management . but be patient such cases may be hard to permanently cure and reach a good physical condition that does not interfere with your daily activities .
Try to send me your MRI reports for proper diagnosis .
