Suggest Treatment For Severe Back Pain After An Accident

Question: I Had a fall at a supermarket nearly 3 years ago. Slipped on a few grapes and my right ankle got caught under a display trolley whereby the wheel lock dug into my ankle and I had a small laceration.. I landed completely flat on the floor on my right side with arm outstretched and wrist hit the floor. My GP diagnosed me with soft tissue damage.
However, after intensive physiotherapy and hydro I starting limping on my right side and my ankle was unstable as it started doing a flicking motion after 1 year I was then diagnosed by a pain specialist with CRPS and after seeing numerous orthopaedic surgeons, the same diagnosis test results were basically clear, except for a small tear of the longitudinal peroneal brevis tendon on the right foot. Further test results were requested by the specialist and I had a styloid fracture and buritis of the right hip along with chondromalcia patella of the right knee..Walking is limited to around 30 metres along with standing for only 10 mins as the burning pain all over foot/ankle increases and is always present.and seems to extend up the leg. I have some slight back pain, along with right sided waist pain with TMJ pain as I also hit my head on the floor and was unconscious for a few seconds. I was offered calcitonin injections. I seeked advice from another pain specialist and was advised it was not CRPS in their opinion but sciatica?? I am now totally confused due to other doctor/specialists confirming the presence of CRPS. This pain specialist ordered a MRI of the lumber spine MCI + STIR L415 TT2, LS1S1 posterior disc, multifidi wasting?... My back X-ray that I had just showed mild spurring.. I would be grateful for an opinion with regards to this, as all the other doctors have confirmed CRPS

I certainly would go with the CRPS diagnosis. This is a fact as what you have, cannot possibly be sciatica, regardless of the fancy investigations asked for by the last pain specialist.


1) You've obviously had a lousy accident.
2) That you had to seek specialist treatment for the same.
3) All this, in a nutshell indicates trauma as opposed to a quick diagnosis of sciatica.
4) Sciatica could possibly warrant a different sort of thinking and treatment.
5) It's my gut instinct and intuition that makes me say this, as I have been treating such patients with a similar clinical profile as yours. Today, fortunately, all of my patients have recovered almost fully, and are not held back by any deficit in functioning.
6) Thus there is a silver lining as regards the suffering you have undergone.

I stand by my diagnosis of CPRS.

And, I personally value your opinion, so I would please ask you to write back as regards what you feel and think, other significant people's views, about this issue.

Being a sports man myself, I have experienced such injuries and I personally know for a fact what you are going through. I would keep away from opiate pain killers like Tramadol, as Heroin is the king of opiates, and all these super pain killers are created from morphine and heroin analogues.

I would kindly suggest that (in order to rid you of the pain)::

1) Take an SNRI drug.
You have 2 choices::
1) Effexor, which is Venlafaxine, it's great for pain and non addictive.
2) Ventab, which is the "sister" of Effexor -- called as Desvenlafaxine.
2) Purchase a TENS device online. You have to use it exactly like a wand to ward off the pain.
Relief is obtained due to the special square wave currents that stimulate the production of
natural pain killers in the body.

I have used these methods on many of my patients for the last 11 years of my experience in my hospital.

So, dear, please supply me with arguably any information that can help me ease your pain, because that's the reason I am here, to help you squash and vaporize the tormenting pain.

Dear Dr Kudchadkar,

Thank you for your reply and confirming in your opinion the CRPS.

The option of the TENS machine has not been suggested to me before until now thank you for your advice..

Could you please advise which brand/or brands of TENS machine you would recommend with a wand as most seem to come with the electrode patches..

In the chemist they have a brand called Ultralieve that has an ultrasound current or Painpod which only has the pads/not the wand and is a TENS machine would either a TENS machine or Ultrasound do very much a similar thing?

The pain that I experience is 24/7 its more bearable when I am not weight bearing, but unbearable when walking or standing whereby the burning pain in the right foot ankle becomes very intense burning pain and accompanies swelling in the area..

Last year in March I had some mirror box therapy by a physiotherapist and was also
doing hydrotherapy during one of my visits at the physiotherapist during mirror box therapy for CRPS of the right foot/ankle the physio rotated my left foot and this caused pain in the left ankle bone. The GP confirmed a sprain and advised me to rest and have no manipulation/or do any hydro the physiotherapist also advised the same.I returned back to hydro in around August last year. but due to the water being cold and it being a bit of a walk to and from the hydro pool I come out of hydro limping on the right foot, exercising seems to unfortunately aggravate the pain on the right side.
Some swelling was also noticed just below the left knee.

In September also this year my CRPS foot/ankle swelled up on driving the car and went into a spasm and on getting out of the car I lost my balance on my right foot
and went over to the left but managed to grab the car door..

Presently, I still have some pain on the left side ankle bone/lower leg area and not sure if it is due to the physiotherapist stirring the CRPS up or that the CRPS has spread to other regions?

I most profoundly apologize for replying so late.
Of course, you need your information.
And I enquired about TENS devices, not the marketing and advertising propaganda supplanted on countless web pages on the net, but wanting the best for you I had to call up people who *actually* use these devices to enhance their enjoyment of life without the shackles of illness.

This is important.

In shopping for your TENS device,
1. Buy any one of your choice and please read the details - we want something that emits "square" wave currents. Alpha Stim is well known in this part of the world, however since the online world is at your disposal at the click of a mouse, you can buy one with bells and whistles - kindly do remember pure "square wave" current emitting devices.

2. Please do not buy a Chinese "Qigong" infra emitting machine instead. Let's first become masters of TENS devices. I respect Chinese traditional medicine, no doubt, and I also respect the advances in medical devices made by the Russians in the Soviet era... we're talking about clones of the "LIDA" Russian machine which has a totally different method of helping you, (By altering your brain electrical patterns).

3. Note well that many enterprising Americans have a DIY (Do it yourself) attitude and have build their own devices in their homes because all the plans for building such devices are legally available to anyone in the world on the US patent office website. I advise you against attempting this for now at least.

4. After buying a TENS device, kindly ask a doctor or an RN how to correctly use it and more importantly what not to do. This cannot be learnt by internet video tutorials nor can it be learn by trial and error. I know at least one patient personally who visited me as regards overuse of his TENS device.

As regards the other part of your question - my serious advice to you right now is to --

1. Learn the Jacobson's Muscular Relaxation Method.

Dear Dr Kudchadkar,

Thank you for making a few calls in regards to the best possible types of Tens machine that would prove helpful. There seems to be a wide selection
of the "square wave" current frequency. As you suggested, I will ask for help in using this device and hope that it will eliminate some of the Crps pain.

Thank you also for the helpful references which I will duly look into and try..
I am a great believer of trying to deal with long standing chronic pain with a positive mind set (although my first thought is to deny and not accept the pain!!) which is not helpful, this was just to get me through each day..

Mainstream drugs do not agree with me and I do not want to add to my issue..
I would rather try alternative therapies or devices that in fact help rather then do the complete opposite..and make a person feel worse. Especially when you try and get off some of these drugs ..

Can you tell me your opinion of the Pulsed RF Blocks? if they in fact work? or can worsen CRPS..
