Suggest Treatment For Severe Acid Reflux While On Pantoprazole

Question: My acid reflux will not go away after two months of pantroprazole 40mg and carsfate after each meal. My diet is also good. The only improvement is that i have an appetite again and i have gained some weight back. How can i get rid of the acid reflux? Should i double the PPI dose? Should i switch to Nexium? Ive heard Nexium is the best.

Hi and thank you for asking!
I read carefully all your concerns and I can say as follows:
1. You are reporting to have had acid reflux and have been treated with pantoprazole and Carafate.
2. To my opinion I would suggest to continue pantoprazole up to 80 mg per day( morning and evening time) plus 300 mg of Zantac on bed time. You can continue to use Carafate and start to use prokinetic drugs like metoclopramide.
If your symptoms will not be improved after 2 weeks than I would suggest to switch to rabeprazole and not to Nexium. Rabeprazole is the newest of PPI's drugs.
3. Meantime avoid spicy and fatty foods.

Hi Doctor,

Thank you for your answers.

How long can i take PPI 80mg? Once i feel better should i cut down to 40mg again? Also, at bedtime, can i take the ppi and zantac at the same time?

I am also taking Mastic Gum in the middle kf the day which is a natural supplement.

Hi back,
With regards to PPI'S you can contiue with 80 mg per day up to 8 weeks and if you will feel better you can continue with 40 mg per day.
PPI'S are recommended to be used before meal (20 min), this is why I sugges to use PPi before having dinner and zantac on bed time.
