Suggest Treatment For Severe Acid Reflux And Chest Pain

Question: Dear Dr. XXXXXXX

As our last conversation regarding to my health issue. in brief in last few month i suffered from gas problem in stomach and due to that sometimes i suffered chest pain. during our last conversion you advise me to check ECHOCARDIOGRAPH cause of in my ECG observed LVH high voltage.
in month of may i was visited to XXXXXXX during that period i tested my blood and did 2D ECHO for health issue.
But again from last 10 days i start to suffered gas problem but when i am in XXXXXXX i feel relax from gas issue in stomach. but here in Thailand i face same problem.
Today also in my stomach upper abdominal slightly pain sometime mostly center of the upper abdominal exact below heart.
But from yesterday feel chest pain, at right and left side and pain like burn at nearer heart.
I attached my report for your reference.
Please advise.

Iron deficiency anaemia and Vitamin D deficiency from reports.

Thank you for asking.

I have gone through your reports. Echo is fine and normal. Blood report shows Iron deficiency anaemia and vitamin D deficiency. Anaemia can cause some chest pain also.
The chest pain you described is most likely from gastric irritation and reflux. Change of diet or using of Monosodium glutamate in foods getting in thailand can be a reason also. Avoid irritating foods and Monosodium glutamate contained foods. Taking proton pump inhibitors like Esomeprazole (Nexium) helps also.
You have to take iron and B12 supplements for correcting anaemia. Vitamin d supplements also should be taken. Iron you can use brands like dexorange which is available in capsules or syrups. Vitamin D is available in soft gel capsules. This you can take 1000 IU daily or 8000 IU weekly. Sunlight exposure is also good.

THANKS FOR ADVISE.

I want to clear about heart disease, this chest pain is related to heart disease?

Such as heart attack or any other disease.

I buy from Market Capical D3 Suppliment for vitamin D, it contains Cholecaciferol Granules IP 60,000 IU. How it a take weekly with Milk? or any other method.

For B12 and IRON i search tomorrow and give detail to you for advise for dose.

Please advise me.

Daily i smoke 8 cigarettes i going to reduce and quit. i tried quit before but i stop one month and again start so i decide to quit slowly by reducing smoking per day.

I know it was very bad smoking so as early as possible i quit it.

Please suggest me on my queries to help us comes out from my heart disease worry.

Your chest pain is not likely related to heart disease as your ECG, stress test and ECHO is fine.
Heart burn and chest pain is most likely related to gastric irritation.
Long term vitamin D deficiency is also a risk for future heart disease. So correcting it is better. You can take 60000 IU weekly once for 1 month and recheck vitamin D. If it is normalized then you can switch off to 8000IU weekly once. You can take it with a glass of milk morning or evening.
Smoking 8 cigerettes daily is also risk for future heart disease. Avoid it if possible or reduce it.

As per your advice i search in market for vitamin B12 supplement. i found supplement as follows:

1. Methycobal 500 ug mecobalamin

2. NURO - B contains are in this tablet
THIAMINE MONONITRATE (B1) - 250 mg
PYRIDOXINE (B6) - 250 mg
CYNOCOBALAMIN (B12) - 1000 mg

so, please advice which tablet good for me. and how to take.

for iron i not get any medicine so please advise for that also.

You can use Methylcobal 500.

You can use Methylcobal 500 mgdaily. After taking it for 1 month you can decrease to once in two days.
For Iron supplement you can take Ferrous fumerate which will be available 600mg tablets. This you can take once daily. Better to take on empty stomach. But if it causes gastric discomfort then take along with meals. 3 months therapy is needed to normalize. You can check your haemoglobin and iron profile after 2 months of intake.
Taking a single dose of anti helminthic - Albendazole 400 is also advised for deworming.
