Brief Answer:
Antifungal + Antihistamine + Topical ointment

Detailed Answer:
Dear XXXXXXX XXXXXXX XXXXXXX

Thanks for your query.

A. Ring worm is basically a fungal infection. Presenting with such symptoms, I usually treat my patients with:
1. Tablet Levocetrizine 5 mg (antihistamine preparation) to be taken twice a day.
2. Tablet Fluconazole 150 mg (antifungal agent) to be taken once a week.
3. Ointment like Lobate GM (pharmacologically Clobetasol + Miconazole + Neomycin) to be applied on the lesion twice a day.
4. Maintain proper hygiene and avoid too much exposure to water or sweat formation in the region.
Since these are prescription medications, kindly consult your Doctor for the same.

B. Recurrence of fungal infection is also an indication of weak immunity status of the body. Certain investigations like complete blood count, estimation of blood sugar level will be helpful to rule out the same.

C. Zimig (pharmacologically Terbinafine) is known to cause Hepatotoxicity in some cases. Since you are on Terbinafine for the past 5 mths, it will be advisable to get your Liver function test done.

Wishing you a good health.
Take care.

Brief Answer:
Minimum one month treatment; follow up after 15 days

Detailed Answer:
Dear XXXXXXX XXXXXXX XXXXXXX

Thanks for your follow up query.

Since you are having the infection from the past 03 years, you will have to continue the medications for a longer duration.
Minimum one month of treatment is needed to get any significant benefit.
Though there will be symptomatic improvement, the complete removal of the infection takes longer time.
I will also advise you to follow up with your Dermatologist after 15 days for a proper clinical examination.
In addition, please get the investigations done as mentioned in my previous answer.
Higher anti-fungals can be prescribed if all investigations are within normal limits.

Wishing you a good health.
Take care.
