Question: I am taking lonazep 0.5 and mirtaz 7.5 every night for last 1 year and 6 months. I was regularly taking alcohol too but were able to sleep 8 hrs in a strech and feeling refresh. For last one week i am not able to sleep more than 4 hrs in a stretch and its resulting not fresh mood whole day. Is the medicine has no effect now or what. I m travelling and will consult the physician soon. I am also determined not to take alcohol and then test the med tonight! I need good advise and treatment.

Brief Answer:
Increase in dose of lonazep can help,it is good if alcohol can be stopped

Detailed Answer:
Hello,

Thanks for writing to us.

Lonazep(clonazepam) is a benzodiazepine medicine used for sleep disturbance-insomnia. Mirtazapine is an SNRI antidepressant also can help in insomnia.

Lonazep and alcohol both act in similar manner in brain. They both cause CNS(central nervous system) depression and induce sleep. Long term use of both leads to tolerance. It means the same dose of either alcohol or lonazep which is useful in sleep disturbance may not be effective after long use. This may be the reason for your reduction in sleep hours.

One other aspect which may be responsible is excessive travelling and secondary stress. Workload can cause fatigue and sleep disturbance.

I appreciate your decision to stop alcohol.Alcohol is overall not useful in long run. Rather it can cause harm to body in many aspects. If you can stop it, it is helpful for your overall health and future. Keep in mind that if you are taking alcohol daily since more than 1 year, there may be chance of withdrawal features after stopping it suddenly. These include tremors, sleep disturbance, uneasiness, anxiety, reduce appetite and so on. Severe withdrawal features can be treated with medicines through help of psychiatrist/deaddiction specialist.

Lonazep and alcohol however should not be combined together. They may cause severe sedation. You are taking low dose of lonazep so you did not have any significant problem yet.

At present you can try to not take alcohol.Increase the dose of lonazep to 1 mg at night until you can meet your doctor. It will help for sufficient sleep.
Taking a cup warm milk before 15 minutes going to bed also helps in sleep.

Try to consult your doctor soon and inform the same what I have described. You need to be evaluated for sleep disturbance and treated accordingly.

Hope I have answered your query, I will be happy to help further.

Regards,
Dr.Chintan Solanki.
