Question: Between 10/2010 and 1/2011 I visited 5 different female escorts having unprotected oral sex (giving and receiving). A few months latter I noticed a mild burning sensation in my urethra that lasted about a week. This happened three or four times until I finally was able to determined that it happened after washing my penis with a perfumed soap. I once tested it by using the perfumed soap and sure enough I felt the burning sensation for about a week afterwards. I stopped using the soap and everything was fine. Between 9/2012 and 2/2013 I visited four more escorts having unprotected oral sex (giving and receiving). About 30 days after visiting the final escort I started feeling a constant mild burning sensation in my urethra which felt very similar to the soap allergy. This burning sensation lasted about four months until I finally went to my GP and he diagnosed it as Balanitis. At this point I had redness around my foreskin and a small area of crouch itch on my thigh. He prescribed 5 daily tablets of Fluconazole 100 mg. A few days after taking the final tablet the burning seemed to disappear. This was around August of 2013. Since that time I have had several episodes of sensations of mild discomfort or mild burning in my urethra. In December I had a painful crack develop around my foreskin which I treated with Monistat 7 and it cleared right up. I have never observed any drainage from my penis. Should I be concerned that this is a symptom of a STD or do you think that it is something else unrelated to a STD perhaps related to yeast? My GP considers it a yeast related issue and does not consider it a STD.

Detailed Answer: Hello Thanks for your query,based on the facts that you have posted it appears that you have repeated episodes of burning in Urethra following repeted episodes of unprotected oral and vaginal sex .over a period of last 3 years . You have done almost all necessary blood tests which have ruled out possibilities of either STD and HIV. Since you had cracks developing in foreskin there is possibility of fungal infection of the genitals which is common in diabetic individuals, Please get your blood sugar levels on empty stomach and 2 hours after lunch done to rule out the same. Since you had recovered completely after taking anti fungal drugs (Fluconazole) there are high chances of fungal infection which has tendency to become dormant for few months and to recur again. Actually only oral sex was unprotected. Any vaginal sex would have been protected with a condom. My last blood sugar level after fasting was 111. I understand that is a little bit high but not yet considered diabetic. I have had a sweet smell in my urine for many years. I wonder if that would promote yeast? Would an urologist be the proper specialist to address the yeast issue? Is this something that can be corrected or should I expect to have occasional fungal infections? Are there any other STD tests that you would recommend that I have not already had based on unprotected oral sex? Thank you for your answers Dr. XXXXXXX

Brief Answer: STD is acquired only by intravaginal sex . Detailed Answer: Hello Thanks for follow up. Std is acquired through intra-vaginal sex and not by oral sex. Since you have carried out all the necessary tests which are reported to be negative you need not do any more tests . As human mouth contains plenty of bacteria it is likely that you are getting this infection due to unprotected oral sex only.
