Suggest Treatment For Red Rashes On The Upper Leg In An Elderly Person

Question: Female, 85, my wife, has red rashes on upper left leg, one shaped like a butterfly I read that Lupus butterfly signs-- rashes occurred on face and hands only. Does the lady have LUPUS just the same ? No 2 Same lady eats very little, so I add some whey to her meals and drinks-- io provide more protein. Is this a good idea ?

she takes 1 mg Warfarin- No 3- What is good to improve her appetite ????---------- No. 4 She has swollen legs-- she takes water pill. Is that all there is to remedy ? P'lease answer all four of out concerns Much apprecisted. XXXXXXX

I've gone through your query and understand your concern for your wife..

Let me get to them one by one..

1. You've read rightly, as this not likely the rash associated with lupus, moreover, lupus has several other features which cannot remain undetected for so long, so no , it's not likely related to lupus.. however a detailed look at it by her doctor is needed to rule out any blood coagulation disorder and/ infection..

2. Yes, you can give her whey, no harm there.. however I'd suggest a consultation with a dietitian for preparation of a comprehensive diet chart according to her BMI, daily requirement of calories etc..

3. The first and foremost thing to improve appetite is physical activity, change of place and surrounding, encouragement to eat, providing her likable foods(as much as possible), keeping varieties of food items so that can provide interest to her.. I can see that she's been through a lot already, so please encourage her to move around with assistance at least three to four times a day for few minutes..

4. I'd suggest you to schedule an appointment with her doctor, as with less eating and so many co-morbid conditions, a few follow-up investigations needed(eg. CBC, Chest X-ray, fresh EKG, 2D-Echocardiography, Liver function and renal function tests etc.) and based on their results, optimization of her water pill as well as cardiac medication..

Now please try to understand that all these are chronic medical ailments and there's only control of symptoms possible along with periodic adjustments of medications as per the overall response of the body to them..

Dr. AB Your answers to the above case seem correct/logical----for which I say thank you. Cannot help asking about one serious problem of mine. I get often enough excruciating cramps up and down both legs. so much so that I cry in pain, sweat and near vomit. Is this a portend of an heart attack coming soon ? Am 90 years old and every thing else is fine. Any of your good know-how ? XXXXXXX old boy.

I believe these queries are regarding you this time..

Yes there are few possibilities, but it's not pertaining to heart attack though..

The first and foremost possibility is Diabetes, if you've already checked for it then okay but if not i'd strongly suggest you to get investigated for it (FBS, PPBS, HbA1C, Urine Routine microscopy)..even though everything is fine, but these are salient features thus need to get evaluated in details..

It can also be due muscle spasm or due to electrolyte imbalance (calcium and magnesium most noticeably) or at times, even nerve entrapment and/ vascular spasms associated problems..to get the exact diagnosis a thorough clinical examination by any experienced physician along with few relevant investigations needed as I've stated above..

Analgesics and pain killers may provide the symptomatic relief for the time being, but a proper diagnosis is required in order to get a proper management..
