Suggest Treatment For Red Bumps And Swelling On Legs

Question: 9 weeks ago my dog stepped on my foot and punctured it. It bled a little, I cleaned it good, put peroxide on it, and antibiotic ointment and it healed. 5 weeks after the fact my ankle and foot would swell each evening after work (I am a teacher, so I am on my feet) but the puncture was fine. Then 3 weeks ago, the foot kept swelling and the puncture got pinkish so I went to the doctor two Mondays ago. It wasn't gout (my dad got gout a lot) or a fracture so they treated me for infection with antibiotics (Augmentin 875 mg 2x a day). By Friday, my foot was very swollen and the puncture was red and raised so I went to the ER Saturday AM. They took a culture but nothing came back because of the Augmentin. Diagnosis was abscess, cellulitis. They admitted me with IV antibiotics: Cefepime in D5W (Maxipime in D5W) and Clindamycin in d5W (Cleocin). I was never in any type of pain. They sent me home Sunday night with Clindamycin which gave me severe diarrhea so I got off of it and went to another doctor this past Wednesday. He put me back on Augmentin 2x day and Cipro 500 mg 2x day. I took three doses total and woke up Friday at 3:00 AM with pain everywhere, tingling and weak muscles, my right eye lid was swollen Thursday morning, and Friday AM my left eye lid was swollen. So I did not take neither antibiotic Friday AM and called doctor. He told me to get off of both and take Benadryl 3 x a day and prescribed an eye drop (neomycin, polymyxin B sulfates and gramicidin). Please note, I am allergic to Sulfa but the pharmacist said it should not hurt because it wasn't going into the blood stream. When I was a kid, sulfa made me hallucinate. Well, after the drops, I was periodically seeing a strange light in the corner of my right eye and a small black dot but it has subsided since I took myself off the drops. Also, the abscess on my foot has not cleared up but it's much better. But backtracking, the night before I went to the ER I noticed what I thought was a bug bite on my leg so I didn't tell the ER doctor about it and I didn't tell doctor because I didn't think much of it. But now I have these red bumps on both legs. 9 on one leg and 8 on the other and I don't think they are bites but they don't look like the abscess either. They are sensitive to the touch, some are flat, some have somewhat of a head, they are different sizes, red, and they don't itch. I am a sixty year old female, 5'6", 156 lbs who rarely gets sick, but I do go once a year for a checkup because I have high blood pressure, and I get my mammogram every year. My last checkup was the same day as the doctor visit for the swollen foot and pinkish area. At that time they put me on Fenofibrate 54 mgs for high cholesterol. This whole ordeal is like a nightmare that won't quit.

Multiple red bumps on your legs could be Erythema nodosum which can develop following Streptococcal infection (?wound infection) and typically resolves spontaneously within 3–6 weeks.
If possible please upload a close up photo of skin lesions on your legs and the injured foot.

I haven't quite figure out yet how to up the photos. But this morning I woke up with a low grade fever 99.8. Also how long will it take for this muscle fatigue to go away? I will try to figure out how to upload the photos

Do you have any co-morbidity like diabetes,hypothyroidism or any other disorder except hyperlipidemia( for which you are taking fenofibrate 145 mg daily ) which can explain your fatigue?
Hopefully you have taken booster dose of tetanus toxoid following the injury and in case your dog is not immunized,any rabies vaccination 9 weeks ago following the punctured wound of your foot by pet.
A repeat complete blood count would be justified in view of return of fever.

Thank you for your query. I can understand your concerns.
I have gone through the photographs .
You have noting but cellulitis with desquamation of overlying skin in your left foot and rest of the skin lesions are either disseminated infection or erythema nodosum.

What you need is prolonged course of antibiotic preferably dicloxacillin and linezolid for soft tissue infection and to check your blood sugar fasting and post postprandial to rule out any diabetes as the skin infection is taking long time to heal up.

I do not have diabetes, my dog was up to date on rabies shot, I did get a tetanus shot while I was in the hospital. My tetanus shot before that was Sept of 2005. I am going to the doctor right now because of the fever.

Thank you for your diagnosis. I think it was on target, however, I did not tell the doctor I went to see your diagnosis because I didn't know if he would like a second opinion from an e-doctor (if that's the correct term). But I do think your diagnosis was more thorough. The doctor gave me a steroid shot and Kflex 4x a day. I think if there is no change in the next 4-5 days, I will seek an internal medicine specialist or an infectious disease doctor. My foot actually looks better than it did when I got the IV antibiotics last weekend, but the red bumps and fatigue are new. Of course I am wound up right now with this steroid shot. But I do have some questions.
1. Why did it take nine weeks after the dog stepped on my foot for the sore on my foot to surface? And why did it surface after it healed?
2. How will the steroid shot contribute to my healing?
3. Is Kflex an antibiotic that kills bacteria?
4. And this is different, I read somewhere that if you eat something sugary while taking antibiotics, the hard to kill bacteria seek the sugar and in turn, get the antibiotic. Is there any relevance to this?

I could see some collection underneath skin peeling off and it can be subjected to culture and sensitivity and also blood culture should be done to know the causative organism -commonly Staph.aureus but by now there may be mixed organisms.

Q1.
a)The causative organism aerobic or anaerobic took its own time and opportune environment to multiply and caused infection of web space of left foot.
b)Your infection was partially treated and hence it resurfaced due to the continued presence of the same organism, suppressed during antibiotic therapy.
Q.2.Steroid shot may help in resolution of erythema nodosum(redbumps).
Treatments for erythema nodosum include anti-inflammatory drugs, and cortisone by mouth or injection.
Q.3 Keflex ( cephalexin) is an antibiotic of cephalosporin group and kills bacteria.
Q4.This hypothesis is unsubstantiated.
