Brief Answer:
Cutaneous vasculitis

Detailed Answer:
Hello. Thank you for writing to us

I am dr.kakkar (dermatologist).
I have gone through your query and i have viewed your concern.
He has cutaneous vasculitis. I can see purpuric rash on both lower limbs.
I would suggest a course of an oral steroid e.g prednisolone 1mg/kg/day for a week.
In addition i would suggest an oral antihistamine e.g cetrizine 10mg once daiky and an oral antiinflammatory e.g ibuprofen for symptomatic pain relief.
He should take bed test and avoid walking as much as possible. Thats really harsh advice to someone on a vacation
He should investigate as soon as possible with complete hemogram, urine routine and urine protein, stool for ova/ cyst, ASO titre, ANA, XXXXXXX Complement levels, skin biopsy and DIF etc.
Cutaneous vasculitus is self resolving if there is no internal involvement. There might be relapses which can be managed similarly.

Regards
