Question: Hi,
My kid is 2year 10 months.He has been going to play school since 4-5months and he did suffer from regular viral fever during this time.It gets fine with medicines.Two months before we had been to our native post that he caught cold,cough and high fever.Again with some regular prescribed medicines he was fine soon but slowly there were some prickly heat kind of red spots around his face,back and hands.As he was active we left those bumps to reduce by itself without consulting doctor and as expected slowly the bumps cleared by itself within a week.

After a month we noticed that everytime he plays very actively because of sweating these rashes got back specially on his nose and right elbow.This again got reduced when I applied olive oil(baby oil) and lotion.Suddenly he developed severe dry cough and the rashes got even severe than before this time around his nose,both the elbows and chin.Then we consulted his regular paediatrician he said this is mild eczema and this could be either because of dry skin or over sweating.But my kid never complained about itching and he never scratches on the area affected.As far as my understanding eczema is mostly linked with severe itching.Also he
never had any problem of such before.I have neither changed anything new to his diet nor I changed detergent,powder,soap.When we consulted the doctor this time he prescribed the below medication.

>First time when he was affected with cough and fever Dr prescribed P250 for fever and LCF,HHlinctus for dry cough.
Post this course he developed red spots around his body but disappeared after a week without any special medication.I suppose the skin allergy was because of HHLinctus which we never gave him before this medicine was new.This happened in July 2015

>Later again in August he developed severe cough this time with no fever but with skin rashes around nose chin and elbows.This time around September1st week Dr asked us to use Macbery for cough and he asked to change the soap ,he suggested daffy baby soap and elovera baby moisturizer.He prescribed emsone for elbow rash and dosetil for face for 3weeks.

The result for elbows is fine,slowly it is getting reduced.But for face it reduces when we apply dosetil but later develops when he plays and sweats.Now he started getting rash on his neckline as he sweats more in that area and the skin around the neckline became rough dark and dry.

But till now my kid never complained about itch.Now we are not sure if this is eczema , any sunburn or any allergic rash.Please give us inputs on whom should we consult and what could be cause of concern.Also suggest few tips on how to prevent or avoid this in future.

To me he seems like an atopic child.
These children are allergy prone which can manifest as frequent cold, cough. They also usually have an underlying dry skin which might develop dry, scaly, variably itchy patches at various body sites.

Thanks Dr. for reviewing my query.I have attached few latest pictures.As said I have been applying dosetil lotion on face since a week I am unable to see the spots on nose.When he sweats and if I stop using the lotion it outbreaks.So I am unable to give you the exact picture on face.

As you can see even on elbow there are small spots now.It was severe before.

I have reviewed the Images.
I can see some redness over neck folds and at elbow. This is a minor eczematous dermatitis and most likely related to underlying dryness of skin which is more common in allergic prone/ atopic patients.
Since you are already using a topical steroid i.e dosetil, I suggest you to continue with it for a total duration of 2 weeks.
You must use a moisturizer regularly for him e.g Atogla moisturizing cream/ lotion, twice daily, all over skin, specially during winters, throughout.
Use a gentle cleansing soap e.g Zyndet soap Or Dove Soap.
Since long term use of topical steroids should be avoided therefore in case of minor relapses of eczema in future Or for maintenance of improvement you may use an immunomodulator e.g pimecrolimus 1% cream (Elidel cream).
The skin condition gradually lessens as children grow older and parents and patients become aware that the skin condition is due to underlying dryness and with regular use of moisturizers they can keep the condition under control etc
