Suggest Treatment For Puffy Dry Eyelids

Question: Hello my brother is 21 years old XXXXXXX living in the UK. For about a year he has started to get puffy and dry eye lids. His lids have also become very sensitive and irritable. His skin is also very sensitive to soaps and shower gels. Whenever he uses regular soaps he gets outbreaks of dry skin and rashes to what we think is eczema. For dry areas around the eyelids we have tried hydrocortisone (1%) but have seen no changes.

Hello. Thank you for writing to us

I have taken note of your query and I have also reviewed the Image.

He has dermatitis of skin of lower eyelid, it is probably seborrheic in origin i.e seborrheic dermatitis.

I suggest him to use a moderately potent topical steroid e.g either fluticasone propionate 0.05% cream Or triamcinolone acetonide 0.1% cream twice daily.

Hydrocortisone 1% is too mild and is not going to be of much benefit.

He must avoid soaps on face and rather use a gentle cleanser for face wash e.g cetaphil cleansing lotion.

For his body he may a moisturizing cream/lotion, twice daily regularly.
Regarding his skin rashes, though it is not very clear in the image, I suggest for him a potent topical steroid e.g clobetasol propionate 0.05% cream, twice daily, over and above the moisturizer.

Soaps are to drying and may exacerbate the dryness and irritation. He may use a gentle cleanser e.g cetaphil cleansing lotion for body as well.

I also suggest an OTC oral antihistamine e.g cetrizine 10 mg once daily in evening.
