Suggest Treatment For Psoriasis

Question: My son who is 21 years old has had psoriasis for the past 5+ years. He has tried everything from creams to shampoos. The psoriasis is now going on his chin and ears. He has seen multiple dermatologists. No one seems to be able to help what would you recommend he could do to help clear his scalp and now his face and ears.

I have gone through your query and I have understood it.

I would like to know which all topical therapies and shampoo he has been on?

Psoriasis is a steroid responsive dermatosis. Scalp psoriasis is commonly managed with topical potent steroid lotions with or without salicylic acid; topical calcipotriol lotion with or without steroid; coal tar and salicylic acid based shampoo.

For facial involvement topical moderately potent steroids like fluticasone propionate 0.05% Or topical Immunomodulators like tacrolimus are usually effective.

Oral treatment with Methotrexate, though indicated for widespread involvement with psoriasis, may also started for recalcitrant scalp psoriasis and for psoriasis affecting limited yet exposed areas of the body e.g face, hands and feet etc.

All these are prescription medications and I would suggest that you visit a dermatologist. The dermatologist will assess the severity of psoriasis and advice accordingly

Hi

The shampoo contains anthralin, and is quite Ok and therefore may be continued.
His prescription medicine contains a potent steroid i.e clobetasol propionate 0.05%, a lotion formulation can be used for scalp involvement, twice daily for 2-4 weeks and is usually effective. I usually use a lotion containing a combination of potent topical steroid with salicylic acid. Salicylic acid loosens up the thick scaling that accompanies scalp psoriasis and this also helps effective drug penetration.
The other cream that you mention contains a very mild steroid. For facial use, I usually prefer a slightly more potent steroid e.g fluticasone propionate 0.05% cream, twice daily.
Oral Methotrexate may be added in addition to the above for recalcitrant scalp involvement not responding adequately to topical treatment.
I suggest a visit to a dermatologist for the needful.
Mind you, Psoriasis is a chronic disease and can recur after seemingly complete cure.
