Suggest Treatment For Psoriasis Of Scalp
Question: Hello,
I have a hard lump roughly 1 inch diameter on the back of my neck just above my hairline. I believe this is my occipital node? Anyway, it has been like this for a number of years, it seems to be very slowly getting lager and sometimes causes very slight discomfort. The lump itself is painless and does not seem to move.
I have also noticed a very hard small lump behind my right ear, another in my jaw line and one on the side of my neck. All have been there for a number of years and are painless.
I also have psoarisis of the scalp. I am extremely tired ALL the time, I never awaken refreshed even after a restful nights sleep.
I quite often have slight iron deficency anemia and my free T4 level is low even though my TSH is normal as is my T3.
I am a 35 year old female with two children.
I am concerned something sinister might be going on or could it be down to the psoarisis?

In a surgical case like a lump,it must have to be examined by a surgeon for its site,size,shape,consistency,overlying skin,temperature etc.
It would be better if you can provide me some details like clinical photograph regarding lump for my better diagnostic view at your one side of neck,behind right ear,jaw line the sites you have mentioned.
It could be lipoma,sebaceous cyst,dermoid cyst,lymph node enlargement or skin warts etc.
Psoriasis might cause pus-filled bumps (pustules) surrounded by red skin but it can not cause lump or does not appear to be like a lump.You can continue your treatment with the psoriasis as here all your lumps at different sites could be separate entity.
So,as I said earlier it must be inspected by a surgeon to make a clear diagnosis.
Do ask me follow-up query with clinical photograph of the lump if possible

I have attached 3 photos, two relaly privide the location one you cab see the lukp. Which is hard and imovable. All have been there for over a year. The one at the back of my neck is in the occipital node. The other two seem to be where there are other lymphs. I am extremely tired ALL the time, with no let up. I habe to keep going as I have two young children but I would much rather lie in bed all day! I also suffer from rhinitis which is troublesome only in the morning. Also joint pain in knees ankles and left hip. I have bout of extreme joint pain averaging ince a month in a knee and shoulder that lasts four hours and is always in the middle of the night which dissapears by the time i wake. I have also had a muscle spasm, my arm jerked up in a spastic movement once. Occasional small numb areas on my face and my right side sometimes seems to droop ever so slightly.

From all your photographs,it seems to be a lymph node enlargement or might be a cystic swelling which is hard,immovable or sometimes painful lump when pressure is applied there.But can only be diagnosed after clinical examination of its size,shape,consistency,overlying skin,temperature,movability etc.
If I were your treating doctor,I would have prescribed antibiotics (penicillin group or cephalosporins or macrolides group) with anti inflammatory and anlagesics medicines like paracetamol + diclofenac combination drugs for short course of treatment.
So,it would be advisable if you could consult a surgeon and get examined by him for proper diagnosis and further treatment.
Rhinitis is mostly because allergic or some times due to viral infection.You can take symptomatic treatment like antihistamines(cetrizine),mast cell stabilizers(montelukast) and try to avoid the contact with allergen.
For your joint pain,you may take over the counter(OTC) analgesics and anti-inflammatory but if you are facing problem since long time then you must see orthopaedic surgeon for routine blood and X-ray investigations to have a confirmed diagnosis.
Muscle spasm or jerks occurred occasionally and once then no definitive treatment required in that condition but if you repeated problem then you can take muscle relaxants drugs with the help of your family physician otherwise three is no need to worry.
Occasional numbness or drooping slightly of one side of face must be evaluated further by complete history taking and thoroughly clinical examination by a physician to make a clear diagnosis but in most of the cases it does not require any treatment and get subsided by itself.
