Suggest Treatment For Prostatitis And UTI

Question: Hello Doctor,
I have been suffering from Prostatitis for last couple of years. Initially it started as UTI without any fever. This was in March 2013. My family doctor prescribed Norflox for 5 days. I felt better after the course, however, always felt a slight pressure in my groin area particularly deep in the base of penis. I ignored it for couple of weeks but the problem lingered on, so I consulted an Urologist. The Urologist asked me to undergo Urine routine test, random blood sugar and Ultrasound scan for pelvis. The urine and RBS test were normal, however, the USG report showed slightly hypoechoic prostate with no evidence of focal lesion. The prevoid urine volume was 340 cc and post void residual urine - 40cc. The Urologist diagnosed it as Prostatitis and prescribed Flotral 10 mg (0-0-1), Gabantin (1-0-0) and Niftran (1-0-1) for 1 month and asked me to follow up with a new USG Pelvis test. After 1 month of these medicines I felt little better but that feeling of incomplete evacuation of bladder persisted. The repeat USG Pelvis scan was normal with slightly hypoechoic prostate. Prevoid urine volume -120cc and post void residual urine---Insignificant. The Doctor asked me to continue with Flotral 10 mg(0-0-1) for 3 months and Gabantin (1-0-0) for 1 month. In addition he precribed two more medicines Bactrim DS (1-0-1) for 10 days and uripas 200 (1-0-1) for 7 days and asked me to followup with a semen culture test after 3 months if I dont feel 100%. After 3 months my condition was same. There was always a pressure in my bladder and feeling of incomplete evacuation. My semen C/s test showed Psedomonas aerugino. I was prescribed Cifran OD 1 gm (1-0-0) for 20 days and followup with semen culture test. After the medication my semen culture test came normal. I was asled to continue with Flotral (0-0-1) and Gabantin (1-0-0) for 2 months and asked to undergo RGU procedure. The findings of RGU procedure was as follows:

"Mild penile urethra shows mild smooth irregularity/minimal insignificant stricture. A small diverticulum of size 13x4mm is noted in inferior wall of bulbous urethra. Findings are suggestive of sequelae of urethritis"

Basis this doctor asked me to take Leotec 250 (1-0-1) for 5 days and told me that there will be some residual symptoms/discomfort for few days.
I felt better for couple of months then again the problem started to persist, this time with an additional symptom of slight burning sensation deep inside the perineum. I was again asked to undergo semen culture test and this time the semen culture test showed Staphlococcus epide. I was put asked to take Bactrim DS for 3 weeks along with my regular dose of Flotral and Gabatin. After 3 weeks my semen culture repeat test came out to be normal and I was asked to take septran along with Flotral and Gabantin for another 6 weeks. I completed the above-mentioned prescription in Aug 2014. I felt a lot better. In fact my symptoms had come down drastically down. However, for last few days,after 10 months now, I have again started to occasionally feel very mild pain in my left testicle. Sometimes I feel slight burning sensation deep in my bladder,feeling of urgency to go to bathroom along with a feeling of incomplete voidation. However, these symptoms are very mild compared to what I felt earlier. Do I have to again undergo all the tests again? Please guide me.

Thanks for your query.Based on the symptoms , investigations and treatment that you have been taking for last one year I can summarize that you have Chronic Prostatitis with repeated episodes of flaring up of infection .

Over a period of last 40 years of my practice I have been treating my patients of Chronic Prostatitis on following guide lines.
1) Do culture and sensitivity of Urethral Discharge obtained after doing a prostatic massage.This helps to get material for culture and sensitivity directly from prostate so that you can take appropriate antibiotics .You will need to take help of a health personnel to do prostate massage and to get material for culture.

2) Do prostate massage every day .For this you will need one of your close relative to get trained by your Urologist .This helps to eliminate prostate secretions every day and relieve congestion of prostate and there by gives relief from symptoms.This has to be practiced for 3 months

3) Repeat culture of urethral discharge after 3 months and if required continue taking appropriate antibiotic and anti inflammatory medicines lie Diclofenac twice daily.

Discuss this modality of treatment with your Urologist .
