Suggest Treatment For Post Peeling Hypermelanosis

default
Posted on Fri, 27 Jun 2014
Question: Hi ,

Earlier had some hyperpigmentation near eyes and on fore head.
To treat that , I did a lactic acid 50% (perfect-image) chemical peel on May 30.
Left the peel on for 3 minutes and washed off.
It burnt and left this hyperpigmentation sort of mark.

Please advise what has to be done.
Applying flutivate and venusia now.
Intend to apply hydroquinone 20% next week.

Appling Melacare Forte plus Kojimax should help.
But will it provide sufficient results.

Thanks

Brief Answer:
Post peel hypermelanosis

Detailed Answer:
Hi,

Welcome to HCM.

I can understand your concern.

I have read history and seen photos. It seems that you have post peeling hypermelanosis.

I usually suggest following treatment in my patients having such problem...

1) to avoid sunexposure, and to use sunscreen gel like sunmask aqua gel during day time.

2) to apply kojic acid containing gel like kojiglo at night.

3) to take tab of vitamin C, glutathion and nicotinic acid.

4) to cleanse the face with facewash containing kojic acid and glycolic acid like ahaglow.

5) to avoid steroid cream like flutivate and hydroquinone containing cream like melacare plus cream.

6) not to use hydroquinone 20% as it is highly toxic.

You may take above treatment under guidance of your dermatologist. I think you may be alright after few months. Still, any query..you are welcome

Regards..

Thank you for your opinion.

Few months seem to be a very long time.
Can we somehow improve on this ?

1. I shall use : Kozimax - this contains 5%Kojic + 10%-VitC. Hence should be more potent.
2. Is it ok to use Hydroquinone 20% just for three weeks. As this is considered the gold standard for treating hyperpigmentation.
3. Will laser help ? Is it commonly available in XXXXXXX ?
4. Please provide specific names of oral medication to be taken. Celin contains only Vitamin C. Any better oral alternative - specifically for a short duration to treat hyperpigmentation.
5. Should flutivate/desowen/momateS be taken or not. As the damage was done only 7 days ago. Maybe it requires some more healing ?
6. Further chemical peeling would be ruled out ? Any other more intense / focussed treatment/application that can be done.

7. I have a black rough mark below my lower lip. This is not caused by the current issue.
It has been there for 5 - 6 years. Dont remember how I got is. Maybe hurt myself. Not sure what it is.
Applied momateS for a few weeks , but no improvement.
Please advise how it can be fixed.

Thanking you

Brief Answer:
Have patience.

Detailed Answer:
Hi,

Welcome back.

You might be improved slowly. You may use kozimax. But, I do not advise to use hydroquinone cream which is highly toxic, it may cause confetti like depigmentation. You may consult laser therapist. But, that also dose not improve in one stting. It may take long time. And there may be side effects also..like erythem, depigmenyation..etc

You may take vitamin C, glutathion and nicotinic acid.. You consult dermatologist for the prescription of these drugs.

Do not apply steroid cream. It might cause atrophy,milia,depigmentaion..etc

Your skin seems sensitive, so I do not advise for skin peeling.

The tough melanotic skin below lip may be nonspecific condition. And tretinoin gel 0.012% may be applied at night and vitamin E cream in morning.

Do not be anxious for the result. Have patience.

I hope you got my answer.

Thanks.
