Suggest Treatment For Poor Impulse Control And Depression

Question: I think I have a worrisome problem with rage. i just lose control at slightest of issues and then I just can't stop my tongue. bi feel ashamed of the language I use. this is not me! it's affecting my daughter. pls help me get out of this horrible behavior i am stuck in. I am a single parent

I can understand your concern. Inability to control yourself and then impulsive behaviour can occur due to impulsive behaviour or due to underlying stress. If the behaviour is present since years and is related to all aspects of life like in personal front, in social front or in occupational front and is associated with uncontrollable anger then most likely this is due to impulse control disorder.

But if this is associated with stress and as you are single mother then the chances of underlying stress of depression are more. There is associated guilt after these episodes which further strengthens the diagnosis of depression. Some times depression may present with atypical symptoms like irritability, anger outbursts and frequent reactions to minor things. In my opinion the symptoms you are having are due to this only but detailed evaluation is needed.

To control that anger both medicinal and non medicinal treatment options are available. Medicines like low dose SSRIs can even help to correct underlying mood disturbance and the frequency of these outburst would reduce. Sertraline or Escitalopram are good options and are safe medicines. Please consult a psychiatrist for prescription of these medicines.

Other option of treatment is psychological methods. Try to relax yourself by relaxation exercises especially Progressive Muscle Relaxation Exercise (PMR) or breathing exercises. This will help to improve your mood. Apart from this Cognitive therapy can also help. In cognitive therapy faulty thought processes, cognitive distortions are corrected by progressive sessions. You have to take help from a psychologist for proper cognitive therapy.

Though the choice of treatment depends upon you but in my opinion combination treatment would be best. So please visit a psychiatrist for help. I hope this helps you. thank you doctor. I agree it's stress I am having. it's not easy being a single parent and for 15 yes. now that my daughter is in 10th she has pressures of her own. I am unable to cope or be there for her since I am bogged down with my own issues.
since I do not have time to go in for elaborate sessions, is there an online version of the therapy?

Cognitive therapy runs in sessions and the therapy requires building of rapport between patient and doctor. It is not possible to administer the online version because this would not cause much effects because individuals usually fail to express their cognitive distortions. So you have to visit a doctor or psychologist for this therapy. Alternatively you can opt for medicines which are safe and these would not waste your precious time.

ok thanks doctor. I will try to make time for the sessions. would like to avoid medications.
I just cannot control my temper. I lose my cool even for minor mistakes my daughter commits. I get frequent headaches. when I do pranayam it helps a bit.
I feel bad I haven't been a good mother. and when I get such thoughts I feel very depressed and I push her away even when she comes to speak nice words to me. I feel bad age has to put up with me. not now not at this age. so I need to get out of this problem.

Considering your circumstances I can understand your problem. Try to accept everything, this is your life and only you can make this beautiful. Try to find out exact events which are compelling you to yell on her. If possible write down on a paper about these events.

Once you write all the issues on paper then make the list starting from most anger provoking events to least. Then start with least provoking thing and try to control anger on that event. Do breathing exercises, or you can take a glass of cold water and take it sip by sip over 2 minutes. This will divert your impulsive outbursts and you will find yourself in better situation. Progress to other issues one by one and deal with one issue over 1-2 weeks. Give yourself time and the impulses will decrease.

Go for a movie every week if possible with your daughter and this will help in bonding.

sure doctor I will visit someone in nimhans.
will definitely use the method you suggested from today itself.
just one last thing. I think it's not just her mistakes. sometimes I get upset with her when actually I am irritated with something at work. that's so wrong. I know it. but I realize it only after I shout at her..
I guess I need a complete change in attitude.

am not sure if it is also my menopausal moods.?

Yes considering your age the mood swings can occur due to hormonal changes in body and this results in increased anger outbursts and irritation without any reason. But for these changes also antidepressants are first choice of drugs.

I'd also advise you to take green tea at least twice a day and this will keep you relaxed.

Start taking Omega 3 Fatty Acid supplements and B12 capsules for better mood.

Take diet rich in proteins and complex carbs and low in fats and this will help to reduce irritability.
