Suggest Treatment For Persistent Pain And Swelling In The Foot

Question: Yes sir, I have had an extended problem with my right foot, It began over 2+ years ago with swelling n the top of my right foot with extreme pain. I have several foot doctors but have received no help. First, I am a diabetic for 15 + years. my sugar has been controlled at times and out of control at others. It randomly swells up and the pain is unbearable. To maloe matters worse i injured said foot in january of 2015, ended up having a severe surgery and loosing half of my middle toe. I have been under a dr's care for the past 18 months and am just now starting to see a resolution in the next month. I have had 75+ hyperbaric treatments and under weekly wound care. They wanted me to offload the foot with a cast but very time they applied a cast the swelling appeared making it extremely painful and i could not stand to continue the cast treatment. Even though the wound is very small now 1/8 inch long and wide I am still having the foot swelling and extreme pain to the top of the foot just as before the injury. I have had multiple x rays, ct scans and mri's. every time the medical report is different. with only vague similarities. Most of which I don't understand. I have no movement in the 2nd toe next to the one that was partially amputated and extreme pain in the top of my foot from the curve of the top of the foot to the toes across the foot. Every doctor has been trying to get the wound on the bottom of my foot to heal but can't seem to get my point to them that this is no the major problem. I have taken intravenous antibiotics 4 times for anywhere from 3 to 8 weeks at a time and over the past 5 months i have been on oral antibiotics(sometimes) 2 at a time)since then. to complicate things more I have condition that requires me to have biologic infusions and i have not been able to have a treatment since January 2015, which has almost caused total debilitation gradually getting worse to this date. I was disabled in 2003 from 2 back surgeries the last one being a fusion at l5/s1. I then had a systemic stroke in 2006 which I have very minor effects from. (slight balance issues). I was on major narcotic pain medication for 15 years but before the original foot pain began I was able to get off all narcotics, and all of my insulin products once i started the biologic treatments. I was a new person with a goal to get off the remainder of the other pills i was taking. Then The foot pain started and i had the accident. My main question is i don't understand the terminology of the MRI's and don't understand what i need to do next.. This is keeping my quality of life in a poor state while have goals of getting back to where i was. I cannot exist without the pain meds (which is far less than I previously had taken . but doctors are not willing to help me deal with the pain afraid i will return to the previous situation I was in. No will take time to listen to what have just written to you and I have no cue what to do to get them to understand. The main problem is I cant understand exactly what the
MRI and other test state. After 18 months of basically house arrest i don't want the original problem to add more time to my situation but it has to be addressed. How can i get someone to look at the past MRi's and other test results and address the underlying problem.
