Suggest Treatment For Persistent And Recurring Acne

Question: Hello sir/Ma'am,
I have been suffering acne from when i was on 16 years now i am 26 years still the acne occurring frequently .I have visited numerous Dermatologist but unfortunately that was proved to be ineffective. I have tried azithromcyin ,clindamycin, benzoyl peroxide and salicyclic acid based face wash and i had also taken 100 mg of isotretinion tablets for 3 months as prescribed by the Dermatologist but those were also in effective when i was taking treatment the acne tends to get disappeared but when the course is get completed within few weeks the acne came out.Here i failed to understand those Dermatologist knowledge to treat acne. I am also doing frequent masturbation which is why the acne are coming because the hormonal fluctuation which i think.how it can be possible to have acne at 26 years of age?
I have a very few questions which are as follows:
A-Why acne occurs?
B-Is there any relation to hormonal level ?If yes then at the age of 26 what do you think that hormone get fluctuated as far my knowledge is concern it is get fluctuated on puberty stage the fluctuation of testosterone hormone.at 26 years of age is tends to negligible?
C-In my family why i only suffering from acne besides me no one ever get pimple?
D-What are the permanent treatment of acne beside which i have mentioned?
E-What are the checkup which has to be followed to detect why the acne are occurring frequently?
F-My skin is oily which cause acne is this true i don't thin so oily skin doesn't cause acne am i right?
If you have the answer then please give in a details manner don't try to give the typical Dermatologist way i am expecting something more because i need to find out why the acne are occurring frequently on me is the any problem inside my body?

I have read your query.
I understand your concern.
As of now, you must be already knowing a lot about acne.
See as you said acne are more common during puberty that is true. But it does not mean that they are limited to age of puberty only.
Acne usually start at the age of 12-14 years and may extend till any age. Infact there is an entity known as Adult onset acne where acne starts appearing later in age after 30.
The pathology of acne is multifactorial.
Various predisposing factors for acne are genetic, oily skin, excess sebaceous glands activity, dandruff scalp, P. Acne bacteria, environmental factors.
So it is the combined effect of various factors that results in acne.
It is not necessary that you definitely get history of disease in your family.
Various treatment modalities for acne target at the sebaceous glands activity , inflammation and against bacteria.
Treatment actually depends on the grade of acne.
So I would advise you to upload your images so that I can actually visualize the lesions and advice you to my best.

I am very sorry to say this i have asked so many question but sadly you haven't given any specific answer.You are here oversimplifying issues.
Regarding Your answer of various treatment i have already mentioned i have tried all the possible treatment which is being given by those typical Dermatologist.
I failed to understand the mentality of Dermatologist here i am raising a question about the knowledge of those Dermatologist i have tried every things so can you describe what kind of treatment you are talking about besides which i have mentioned give me the details.
As you said adult onset acne which onset after 30 is the tends to be negligible so we can consider the lesser number of case here so it is the baseless argument.
If you know something different treatment of acne then give the answer otherwise i have already know those recited type of answer which are proved to be in effective and it does take so much of valuable time as well as money.

What grade of acne you are talking about ? I want a permanent solution give me the answer that i will not have acne because at my age all my friends colleague and other relative i haven't come across anyone not even a single who are having acne.
So my question still remain same why me?
Here I am trying to figure out reason as well as permanent and very expensive kind of treatment as well as expensive medicine and expensive creams.

I am expecting a lot more from you this time so give me the details as much as you can

Hello. Welcome back.
Grade of acne depends on the the type of lesions and severity of lesions.
This will be decided by me after looking at your lesions. There is no standard treatment for any disease. The treatment is decided on the basis of the type of lesions.
For example, I prefer to do Radiofrequency to ablate the comedones which is one of the of the stage of acne.
For papules and pustules, I prefer to advise oral antibiotics like doxycycline / minocycline to my patients if they dont respond to it then consideration is given to start oral retinoids like isotretinoin which you have already taken.
But 6 months course of isotretinoin is mandatory to avoid recurrence. Once it is completed, I advice my patients to apply topical retinoids to avoid further eruptions even when face is clear.
I guess you stop the treatment abruptly.
Fractional laser and Nd:yg laser are also helpful in treatment of acne.
Besides this, chemical peels like glycolic acid, salicylic peels are done to prevent recurrence. These help in reducing sebaceous glands activity thereby decreasing chances of recurrence.
We get to see a lot of patients of your age group getting acne.
Avoid hair oil application and oily food in the diet.
Wash the face twice daily with soap and water.

I have seen the attached photograph.
Your face looks quite clear. There is definitely some eruptions and pigmentation. But it is not so much as you were worried.
Do not worry for occasional and few eruptions.
Still if you feel that the acne aggravates in between then you may apply topical retinoids like Retino A 0.025 percent over the face to prevent further eruptions. It may make your akin extremely dry. For this, you need to apply liberal amount of moisturiser over face multiple times a day.
Alternatively you may take Nd:YAG laser
sessions from an expert Dermatologist.
Chemical peel preferably a combination of Salicylic acid and Mandelic acid will help you in controlling acne and improving hyperpigmentation.
Avoid hair oil application.
Wash your face twice daily with soap and water.

Hi!
Thank you for your quick response but i would like to differ with you on some point on your salicyclic acid/Mandelic acid based peel as both of the peels makes the skin quite dry and required mutiple sitting
For hyperpigmentation triple combination cream is very effective which is also know as bleaching agent hydroquinone 3 to 5% supposed to be a good options but you didnt prescribed for that.
What you saw on photograph was pimple related scars and combination of hyperpigmention.As far as my knowledge is concern for pimple scars fractional co2 supposed to be a good options but you didnt prescribed for it.
Combination of kojic acid and vitamin C also treated to be good lighting cream along with vitamin C tablet for oral suspension yet you didnt mentioned it.
Anyway i keep on washing my face twice regurally with Cetaphil face cleaner along with Ahaglow and Glogeous face wash .Along with i have tried numerous suncreen lotions Starting from Ansolar,Photoban,suncross,which were quite ineffective.
Whenever i visited to a Dermatologist they only precribed only cheap suncrean and moisutizer which were below Rs 1000 i failed to understand you people didnt have idea about expensive skin care treatment i really got fed up such attitude if those doctors.So if you know any expensive night cream,sunscreen lotion,facewash,moisturizer do share with me which will give better results too

you may use Sebamed brand products

You may you Sebamed sunscreen, Sebamed face wash, sebamed moisturising lotion. Sebamed is a very good and trustworthy brand.
Fractional laser is far better than CO2 laser for XXXXXXX skin. You may go for it also as I mentioned already. Definitely peels make skin dry but they are quite effective too.
One may use any non comedogenic moisturising lotion to counter part the dry component.
In night you may use kojic acid or glycolic acid based cream. Kojic acid would take care of only pigmentation part but glycolic would target both acne and pigmentation.
