Suggest Treatment For Panic Disorder

Question: Hi, Doctor 2 years back at midnight.Suddently some tingling effects happened in my whole body and I fell down.This time I was a difficulty of breathing.I taken to the hospital and after sometime everything got fine without any medicine.After this incident again in 2013 same tringling effects happened suddenly and my right hand and face feeling numness.I taken to the hospital .They taken my CT and said no issue .This numness happened again in different intervals.After one day it;s started for my left hand also.When its comes it;s starting slowly and getting to peak and again it;s reducing.This time my pressure will go up and mentally I am totally down.I have taken MRI and EEG and doctor said no problem .You have only anxiety they suggested to take exilor . After this no issue .But again after 2 ,3 months I started suffering different feelings .Sometimes I feel pain in my back side of head,sometimes right hand feeling numness.some unusual feelings in heart. etc .All this times bad things are coming to my mind and I cannot do anything I become totally down.Agan I went to another nero surgen and he said this is type of fix and told me to take trioptil .But no changes .if i am ok in 4 days then 2 days I am suffering this. Last I visited another nerologist,they said it;s a kind of migrain and started medicine Dicorate Er 250,Amixide -H,Exilor .4 months there waas no issue. But it's stared agian from last week .. Once i recovered from this problems I am totally ok and no feeling of happened like this. When it's happening I am feeling like something bad is going to happen to me and my thoughts are un controllable. I am working as a computer programmer .Because of this issues I cannot work properly I have lot;s of family tensions . Please advice me what is my real problem and how I can get rid of this . I stop all the medice from january 2014

I think, you are suffering from panic disorder. It is an anxiety disorder characterized by recurring severe panic attacks and these attacks typically last about ten minutes or less than that, but can last twenty minutes to more than an hour or until helpful intervention is made. Common symptoms of an attack include rapid heartbeat, perspiration, dizziness, dyspnea, trembling, uncontrollable fear such as: the fear of losing control and going crazy,the fear of dying, headache, tingling sensation in arms and hyperventilation. Due to number of attacks, patients usually remain apprehensive and that lead to further attacks. Same problem is with you. In that case, you should consult a psychiatrist rather than neurologist. In treatment, you need antidepressant like paroxetine or low dose benzodiazepine that would help to control further attacks. In case, you need further help, you can ask. Thanks for the reply doctor I have some more quiries.what is the reason behind this dicease . Because before 2012 i never had this type of issue in my life. Secondly if i take the medicine as you prescribed how long i have to take and what is the side effects. Can i stop this medicine by changing life style ? If this disoder continues long term without any medicine what will be the result?

Brief Answer: Antidepressant would help to control symptoms Detailed Answer: HI As i told you that panic disorder is a anxiety disorder, so it happens due to underline stress. If you have any stress in your mind, it could lead to anxiety disorder. The medicine, which i have mentioned, is a antidepressant and you have to take it for months, rest depends upon response with medication. It has some side effects like delayed ejaculation, nausea, gastric problems etc, but these are not major side effects. You have to consult a psychiatrist for this drug. It is not available over the counter. You can also try some relaxation technique like yoga, meditation, deep breathing exercise etc. Life style modification would definitely help you, so do it. Doctor my last question Before taking medicine is it better to try the life style changes ? like taking some rest from current job , yoga, medication etc... Also if this dicease untreated for long time what will be the effect?

Brief Answer: I would suggest medication and life style mod Detailed Answer: Hi Psychological disorders happen due to underline stress and life style modification would help you to decrease the stress. You can try life style modification before drugs. There is nothing harm in it. Panic disorder is not life threatening, it just lead to socio-occupational dysfunction. Sometime due to panic disorder, patients stop going away from home or job. This could lead to avoidance tendency and increase the frequency of attacks. Some time, it lead to generalized anxiety disorder, in which patient remain anxious throughout the day. To avoid all consequences, better to take treatment and spend normal life.
