Brief Answer:
urinary tract infection

Detailed Answer:
HI, thanks for using healthcare magic

It is likely that he has a urinary tract infection.

A urinary tract infection can involve any aspect of the urinary tract- kidneys, ureter, prostate, bladder or urethra

Based on the fact that he is experiencing difficulty in initiating urination - it is likely that it may be the prostate- prostatitis

He would need to consider visiting his doctor for assessment.

This assessment would include physical examination of the abdomen, urine analysis.
Imaging may or may not be needed.

He would need a course of antibiotics.

Summary: urinary tract infection, possible prostatitis, would need antibiotics

I hope this helps, feel free to ask any other questions
