Question: hi doctor thanks for taking my question. late 50s male, with a pain in the a**! diagnosed with a fissure by a np, referred to a colo-rectal surgeon at florida hospital, he said it was a rash, and that after doing poking and prodding, said there were no in or ex hemorrhoids, no fissure, but there was a nasty rash between my cheeks. the pain can be quite debilitating. GP did a culture, came back bacterial, now on oral erythromycin and clindamycin with zinc acetate cream topically. it's cure me or kill me, it can get so overwhelming pain wise. i tried the erythromycin gel topically but the pain was off the charts. do you think i'm on the right track? i can't take much more of this! now of course the oral antibiotic is making me go more often, which aggravates the pain, and makes me a very grouchy person. any advice? thanks SO much for your help! best regards, XXXXXXX

Brief Answer:
as explained in details.

Detailed Answer:
Hi.
Thank for your query and an elucidate history.
To recapitulate: Male/50 - diagnosed as rash in between butt cheeks - taking and applying medicines as mentioned. Wants to know more...

Please give some additional information:
- Since when have been suffering from this?
- Please post clear photographs of the affected area as this sometimes helps me for on-the-spot diagnosis.
- Any other medical problems that you have like diabetes, intestinal problems or any other disease?
- Any other information that you would like to share?

My thoughts:
As the diagnosis has been given by a Colo-rectal Surgeon, this is a local dermatitis of the skin of the inter-gluteal region.
I would advise you the following in such a condition:
Wash the area with plain water 2 to 4 times a day and make it dry with a cotton cloth or hair dryer.
Apply Solution Povidone Iodine to make the area sterile and let it dry up.
Use loose underwear and pants.
Avoid sudden changes of different temperature zones as it instigates sweating , which in turn enhances the dermatitis problem.
When at home, keep the area well ventilated.
Continue the antibiotic and add on probiotic which may help restore normal bowel movements.
Add on multivitamins as they act as an anti-oxidants.
Get Vitamin A and D estimates and get supplemented as per the reports, these are most needed to maintain the integrity of the mucosa and the skin.
Ask for Metronidazole orally as it helps to tackle atypical bacterial infections. Will be given if your Doctor feel the necessity.

With these measures it is possible to have a proper healing and avoid the recurrence. This helps most of my patients.
Please feel free to ask for further relevant queries if you feel that there is a gap of communication.
