Question: Painful joint on hand, skin rash terrible itching. Seems to be healing then roars back. Supperating spots in circular pattern. No fever, not spreading to other places.

Hello,

I think you're suffering from rheumatoid arthritis. The rash of rheumatoid arthritis doesn't itch but maybe you developed interstitial granulomatosis.

Interstitial granulomatous dermatitis is another rash that can occur with rheumatoid arthritis. We also call this condition rheumatoid papules. Symptoms associated with the condition include red plaques or bumps that closely resemble eczema. The rash is itchy and often painful. However, interstitial granulomatous dermatitis is very rare in people with rheumatoid arthritis.

You should be checked for rheumatoid arthritis by checking
* Rheumatoid factor, ccpl
* Eritro sedimentation.
* PCR
* Total blood count
* Hand joint x-ray
You can take anti inflammation drugs such ibuprofen to ease inflammation.

Also can use topical ointment with antibiotics and corticosteroids (betamethasone gentamicine) to prevent infection and reduce inflammation. Your doctor will evaluate situation and with treatment for arthritis the rash will be treated too.

Thank you so much Dr Xhufka. I am going to see my physician tomorrow and I am dreading it because I know you are right. This represents the end for me - I see so many physical problems that could emerge, and I am looking at a much shorter life and all kinds of difficulties, as my mother had it very badly. I only hope that they have invented better drugs that won't harm my liver. Thank you again for your very clear and thoughtful response.
