Question: Hi Doctor, I am a 63 year old male who is experiencing pain with my penis foreskin when it retracts to have sex. What would you recommend to lessen the pain?

I read your question and i understand your concern. With age of 63 and difficulty of retracting foreskin and pain on coitus indicates penile foreskin adhesion with likely infection like phimosis and paraphimosis etc.

I would suggest conservative measures to consider first like manual reduction, osmotic methods, hyaluronidase application , vertical incision etc for the retraction and topical hydrocortisone etc for the relief. if not then i am afraid you will have to go for circumcision for therapeutic reasons and relief from this adhesions and pain associated with it. Talk to your urologist and let them decide what is best for you.
