Suggest Treatment For Pain Near The Appendix Area

default
Posted on Fri, 25 Sep 2015
Question: I've been getting pain in the area of my appendix, that's in the general area where & small & large colon meet.
I told my doctor about it & she ordered a test to check my abdomen. I told the technician that was doing the test that the pain was in the lower right side of my abdomen. So she did a quick scan of that area & went back to my abdomen (which is what the doctor had instructed). Anyway whenever I eat bread, cereal, pancakes or whatever that has wheat in it, I'd get these cramp like sensations in the lower right of my abdomen.
Checking on the internet, it appears to be cholitis (not sure of the spelling). I hate self diagnosing myself, but that was the best I could come up with. I'm an accountant, not a doctor after all!!
It's been going on for about 2 months now. I had a colonoscopy and mentioned it to that doctor, but he seemed not to be interested in my complaint about my lower right side of my abdomen. He was concentrating on his primary task - the colonscopy. He removed one pollup he said. He did not say anything to me that I should be more selective with my food choices. That info I got from the internet. So I have started watching labels and have started purchasing gluten free pasta, for example, and try to be more aware of what could be triggering the discomfort I feel on my lower abdomen on the right side. What's my next step? Who do I go see? Is gluten intolerance a sign of something else? I end up putting a heating pad on that area until the discomfort subsides. What does a doctor need me to explain
so that he can diagnose my problem? How does one seemingly suddenly become gluten intolerant?
I used to drink a lot of milk and eat other dairly products. I cut back on those items too. Is there a cure or is this just a process of getting old (I'm 68) and have been relatively healthy all my life. I'm 5'7" and weigh 177 pounds. This getting old is for the birds!! Thank you in advance for your reply.

Detailed Answer:

HI,Dear,Thanks for your query to HCM Virtual Clinic.
I read and reviewed your very elucidate query and understood your health concerns.

Provisional Impression-Right Iliac Abdominal Pain?Amoebic Typhlitis?Under Investigation.

Your query is very complex and would take 1-2 more sessions before I go ahead to give you exact answers to all your queries.
I would still try to answer your queries one by one with very limited clinical information from your doctors notes or your earlier medical documents.

I would need following data-
What is your stool test report?Stool By concentration method
is done or not?
Is there any stool mucus/ and occult/fresh blood /pus cells/Ova /Cysts/Protozoa reports?
What's your latest Colonoscopy report, update its copy if you can?
Did you had any lower gases/cramps off and on lately in last 6 months?
How frequently you eat outside?
What's your daily life schedule, of work /diet and rest?
Update any other report/doctors notes in last 1 yrs or so?
In the meanwhile, till I get your above updates,I would explain the queries asked from you with whatever extent I can as follows-

My observations on your query-
-Don't be panicky by the facts read from internet,as they may increase your anxiety and has complicated your issues more for YOU and for your doctors too.
-Let the doctor's work in their way and style.Don't expect immediate answers from them,as they would definitely help you and working rightly for your case.
-As yet your diagnosis is not confirmed and its reasons are being worked out.
Doctors are working with any other hidden serious aftereffects of your informed illness, which is a challenging and very responsible job for them.

-About your diet-
Reduce on spicy /fried/non-veg red meat diet.
Take bland liquid diet of little quantity for 3 weeks
Be on pure rice, dal, ghee with a moderate fiber simple diet.
Reduce on wheat for next 3 weeks.
Be on Butter milk 2 times a day.
and Banana Milk Shake 1-2 times a day.
Spinach soup with less of chillies and spices x 1- 2 times a day x 3 weeks
As rightly said by you,Gluten intolerance doesn't develop in 2 months time.
There are some issues in your small and large bowel and are being worked up by your doctors and also by ME.
So don't be panicky and anxious on gluten free diet, but definitely reduce on wheat volumes and be on rice supplements with minimum daily volumes.
Get sufficient fluids and water to hydrate for your physical life/work style?I would comment more in next session after I get your above detailed updates.
This would stabilize your unstable colon and intestines to stability;

Medicines suggested under cover from your ER doctor/PCP,

-I would suggest you to be with Gastroenterologist and Abdominal Surgeon team, who are the right doctors for your case.
-Update your USG/Colonoscopy/Stool reports for more detailing on suggestions.

-Antibiotic+Anti-amoebic drugs(like Ofloxacin+Ornidazole) 2 times a day(after stool examination by concentration method ONLY-to avoid missing the cause of this right abdominal pain/discomfort)
-Antispasmodics(Cyclopam like)
-Multivitamin(Like Tab-Zincovit +Nutrolin B)
This would stabilize your health.

Follow up: Dr. Mukund Savaskar (15 hours later)
Thank you for your reply. I'm going to contact my doctor and the gastro dr who did my colonoscopy and follow up with them.

Since I wrote you, I did a little more exploring on the internet. I made an error, I believe I may have celiac disease. I do get tired in the middle of my day, and as soon as I lay down I am asleep. A couple of people have seen me pass out like this on occasion, but when I'm tired I don't have any problem falling asleep - immediately!!

I will also follow your dietary suggestions and see if that will help eliminate the discomfort I have been experiencing. I have not had any of the tests you inquired about, but I am going to follow up on those & will seek your counsel and opinion about the results. So it may be a couple of weeks before I get those results depending on when I'm able to make an appointment with my local doctors.

Thank you so much for your prompt reply. You sound like a very caring physician.
I wish you were in the XXXXXXX area. Sincerely, Arleen XXXXXXX
