Suggest Treatment For Pain In The Testicle

Question: HI there
I have had pain in my right testicle which started 2 days ago when I was laying in bed at night. It was sudden and caught me by XXXXXXX I was moving from my left side to right side in bed and it was instant pain. Lasted about 5 to 10 seconds and then calmed down after I got into a confortable position where my testicles were more "free". Also, my right leg feels as if the muscles are irritated and I feel like squeezing my leg. This started at the same time. The pain in the testicles is only if I try to squeeze it, or if I push it. In other words, It doesn't hurt at all if I am just sitting here or laying down. I need to touch it in some way to feel the pain. The pain is mild, more like a soreness but I can only feel it if I purposely try to squeeze the testicle (by squeezing legs together, or sitting cross legged). No other symptoms. I've been to a doctor who does not seem too concerned. He did a physical exam. This is not the first time for me. But almost always, the pain would come suddenly and go away within about 10 seconds. The testicle would then be sensative for a little while but nothing major. This time the pain, again was only a few seconds, but the sensativity has lasted longer and if I interfere with the right testicle by pushing or squeezing it, that hurts a little bit. Otherwise no pain if I am just sitting, standing or walking. My right leg has been hurting on and off though. It feels as if I might have pulled something suddenly when I turned from left to right side that night. I am more worried about torsion. But the doctor said torsion pain even though severe, would be lasting for much longer and I would not be able to ignore it. If it is torsion though, it has been 2 days and I am very worried because by this time there is no way to save the testicle right? But the actual sudden pain in that area was a few seconds. Now it only hurts a little bit only if a squeeze in on purpose. When doctor examined it he did not notice any swelling, lumps, rashes, he said he also checked for hernia. I asked if I should go to emergency but he said there is no reason to. For now, I am waiting for ultrasound appointment. I would like a second opinion as I am very tensed. The sudden pain is the only things associated with torsion that I also had in common, but as I said, only for a few seconds.

Thanks for your query and an elucidate self-explanatory history.
Read your history carefully, about a Doctor's opinion, repeat of the same a few times indicating your anxiety and stress about the pain which is not yest diagnosed and so on.

My thoughts from the history that you have provided:
- Torsion pain is very severe, unbearable, keeps one tossing in bed without any relief for much longer time as your Doctor has explained you.
- Local examination also has not revealed anything serious.
- Sudden pain of a few seconds can be due to pinching of the nerve along its course due to tight muscles of a young 22 years old man, stone or infection in the ureter and so on.

Hence I would advise you the following in such a situation:
- Urgent ultrasonography and color doppler of the scrotum and of the abdomen, particularly the genito-urinary system to rule pout torsion so that your mind will be at ease, relaxing you and the muscles. This will help to get an appropriate diagnosis by exclusion or confirmation.
- Tests of blood, urine and as required.
- An opinion of an Urologist may be required.

This way a proper diagnosis can be made and a plan for management can be had.
