Question: I'm a 34 yr old woman. starting @ age 13, anytime the top of my butt crack is touched even lightly or my butt cheeks r parted (washing or wiping etc), it feels like the tissue beneath the flesh is tearing & the intense searing pain lasts for hours & I can't sit, bend or squat or anything that might part the cheeks in any way. docs say NO cyst, NOT scyatic nerve prob & r @ a loss. 20+ yrs of this issue. no injury in or around area ever.

You are talking about pilonidal sinus. The deep of muscles on the butt opens outside to the skin making it a sinus and very hard to heal by itself. If this recurs which it usually is it cause such intense pain, may bleed and infect as well.

The best way to prevent some recurrences to happen is by slitting it open by surgeon and letting it heal secondarily. With this treatment the healing happens according to a plan called secondary intention. This is regularly employed by surgeons in their clinics. I think you should talk to one in your area about pilonidal sinus surgery. There is thick fibrous tissue that forms over this area and the frequency of cracking reduces.

Other ways of treatment may not help.
