Brief Answer:
Ultrasound should be done.

Detailed Answer:
Hello!

Thank you for the query.

Is the "knot" located in the soft tissues of the breast? If so, you should definitely have an ultrasound of your breasts done as this knot can be a breast tumor.
If the lump is close to the collarbone, enlarged lymph node can be the reason. This needs an ultrasound as well to diagnose the problem.
If you smoke, also chest x-ray should be done.
If you have fever, infection is the most probable reason, and the knot can be some kind of inflammatory lump.

Hope this helps.
Regards.

Brief Answer:
X-ray should be done.

Detailed Answer:
Than most likely it is not related to the breast. If its a hard knot, it might be some bony lump. X-ray should diagnose it.
