Suggest Treatment For Pain In Testicles

Question: Hi Dr Aniruddha,
I have slight discomfort/mild pain in my right testicle so i prefer to consult urologist. Upon Scrotum Ultrasound i diagnosed with mild left varicocele with no problem in right testcile and asked me to wear scrotum support. It might be pain/discomfort in my right as few days back something got hit in my right testicle. Both R/L testicle and epidydmis are normal in size and i feel no pain/discomfort in my right testicle. Also my right testicle move more up and down than compared to left. Also left part slightly hangs lower than right. I undergone semen analysis test . I am attaching scrotum ultrasound summary and semen analysis report. Please let me know if i can be father based on your report. Right now am unmarried, please let me know if i should get married and become father. Also predict possiblity of becoming father in an year after marriage. Many thanks in advance.

No need to operate for varicocele

Hello,

Let me explain the reports you have attached in detailed :

1) According to evidence based science there is no need to operate mild varicocele which was identified on ultrasound scan only.

2) Movement of testis in scrotum is normal and it is normal to have left testis hanging is normal. This is normal. Pain you are experiencing is not related to varicocele.

3) Your total motility is 75 % ( Rapid 25% + Sluggish 50%) and this is normal.

4) Your count is excellent .

5) These parameters are more than enough to father a child within a year after marriage.

6) You dont need popson medicine as your semen is normal.

7) You dont need surgery for varicocele at all.

Hi,

I am having discomfort/mild pain on right testicle while left varicocele was identified by ultrasound. Right is perfectly fine as per ultrasound and both are normal in size.
2. What should be done to recover this discomfort on right testicle?
3. As per your comments i can get married na?
4.Is one testis sufficent for sperm production?
5. In what cases varicocele should be operated?
6. Can mild varicocele lead to testis failure?

Hi,

1.Pain in right testicles could be due to tight undergarment or pants.

2. I suggest you wear loose pant and no undergarment for next 15 days. You will be fine.

3. You can get married comfortably and have children. No problem.

4. One testis is enough for sperm production.

5. Varicocele if affecting semen analysis or large & clinically palpable should only be operated .

6. Mild varicocele does not lead to testicular failure. Its nothing.

1. Should scrotum test be done in morning/afternoon/evening as scrotum size varies in a day. The doctor took me 2000 for this test and did it morning and now again he is asking me to repeat this ultrasound as he said it should be done in evening. Please enlighten with your supreme knowledge.

Size of scrotum does not vary so much in morning and evening so as to repeat it in evening. Your doctor is probably wrong.

Even handwritten report does not seem convincing.

I think your doctor is not convincing. You can ignore the report about varicocele.

Many Thanks for your advice and suggestion.
Can you let me know the reason for this varicocele or things to be taken care off so as to avoid this. Also my right testicle move more up and down than left, is it a normal case.

Even i think my doctor is not convincing and misleading me to make his money. He suggested me that Rapid motility should be close to 60% and count should be around 60-100 million/ml. Is his knowledge absurd? i have the handwritten report and ultrasound picture, do you want me to share with you. He also suggested me that we can go for ICI and IVF if popson will not work.

How much count is expected? and what should be the rapid motility rate? Please let me know as am about to be married next year.

It is normal that males have left testis lower than right.

According to WHO 2010 guidelines, your semen analysis report is normal. Your doctors are misleading you probably.

Normal count is 15 million per ml. Normal progressive motility including rapid and sluggish is 40 percent. You have more than this.

You can become father normally. No need of IVF
