Suggest Treatment For Pain In Testicles

Question: Hello,

I'm a 30 years old male with pain in my left testicle, specially while siting, crossing legs or with the pressure of underpants and trousers. I'm working in an office, siting 8 hours.
Background:
I remember little discomfort on the left testicle using tight underpants or using climbing harness when I was 16-24 years old.
When I was 25, I was using an old bicycle with a damaged seat, when I braked the bike and my testicles scratch on it, and I felt pain. Since then the pain has reduced to discomfort, but some periods of time (months) becomes painful again. It depends on if I have had pressure on my testicles (e.g: like if I have seat in a convex chair). Now I have pain for 3 months.

Hello
Thanks for your query,based on the facts and reports that you have posted it appears that you have mild Varicocele(Grade 1) with Epididymitis.

Ideally there is no treatment for mild Varicocele except taking pain killers ,The result of the surgery are not guaranteed and usually advised in grade 3-4 Varicocele.

Use scrotal support by reverting the scrotal sac and put soft folded cloth(Handkerchief) over it and wear loose under wares .This will help to relieve pain by avoiding dragging of the dilated veins.

If discomfort is untolerable you may take pain killers like Diclofenac twice daily.

Thanks Dr. XXXXXXX for your diagnosis and advices

According with your information, the following questions come to mind:

1- "Mild Varicocele with Epididymitis" means that the varicocele is causing pain at epididymis? Or means that I have a varicocele and, separately, epididymitis?

2- Do you think that could be possible that I have a damage (caused by bike) at epididymis for 5 years?

3- Family doctor prescribed Diclofenac (Voltaren), and I just feel a very tiny improvement, and also my ears start ringing louder (I have tinnitus for 10 years). ¿Is there any other treatment, like apply head or cold?

Thanks for follow up.Varicocele and Epididymitis are two different entities and Epididymitis can not not be due to Varicocele.

Cycling will not damage Epididymis .

You have to use scrotal support as I have advised in my reply earlier and continue taking Diclofenac twice daily minimum for 3 weeks .
