Suggest Treatment For Pain In Testicles

Question: Hi, I am 32 years old and unmarried, I am having mild pain in my right testicle for last 3 months. The pain start few hours after ejaculation and stay on for few hours. It sometimes radiates to right leg. The pain also increase when I press break multiple times while driving my car. I have seen a Urologist already, he said there is no major problem. He gave antibiotics for 2 week which is done. I went through ultrasound of scrotum, semen culture & analysis but there were no abnormalities. But I am still have testicular pain which causes discomfort.

Have you checked for Varicocele ( dilatation of veins more than 2 mm in color Doppler ) ? This could cause symptoms what you are describing? Please get a colour Doppler for the same.

Hello,

I have already gone through color Doppler of scrotum. The report says as follows:

Both testis are normal in size & echo texture (size Rt.33x18mm & Lt.35x17mm)
No focal lesion, no hydrocele, no varicocele, Epididymis is normal
normal blood flow in both testis
no evidence of torsion
both spermatic cords are normal.

my doctor was suspecting partial torsion and asked me to do the ultrasound again when I am having pain. Do you think this is necessary?
I have noticed that the middle line separating two testicles is not exactly in the middle, it is more towards right testicle when relaxed. However when excited, it comes in the middle.
My pain is not dependent only on ejaculation. It increase if I sit cross leg, when I sleep on my stomach and put pressure on my testicle, it again increases.
If I am hungry for ex. if fasting, it again increases.
sometimes I feel it could be a muscular problem. Do you think massage etc. might help.

I do not think massage is going to help. it does not seem to be a serious problem. Itcould be orchitis due to non specific infection or due to psychogenic origin. Do you worry a lot over small matters?

I don't worry much on small matters. My doctor also told me that there is nothing to worry about but since the pain is not going away, I don't want to ignore it.
One more detail I want to give is that I am suffering from Anal fissure for almost 2 years. The fissure is not severe though. Can this cause some sort of infection?
Regarding Orchitis, I have taken more than 2 week dose of Orni-O, so this should have resolved.
But this problem has restricted my sexual activity, libido is more or less same but I just try to control it.
If you think this is psychogenic origin, what would you prescribe?
