Suggest Treatment For Pain In Groin, Testicles And Lower Abdomen

Question: I have an issue in my groin, lower abdomen and testicle region for almost 2 months now. I have gone to both an Urologist and my family doctor I see regularly. I have also been a Type 1 diabetic for 25 years, so I am use to having health issue arise. About two months ago, I started having intense pain in my left testicle and the region just above where the penis comes out of the body. It feels like the pain from being kicked in the balls, but its there constantly, like something is squeezing it. Than pain also transfers to the lower abdomen area. If I lay on my back, the pain typical dissipates. However, when I am sitting at my desk and leaning forward or slightly forward to the side, the pain becomes intense again. Leaning forward in the sitting position causes the worst of it. I have gone in for a CT Scan already and it showed no tears, lumps, growths or a hernia. My family doctor prescribed me a months dose of Doxycycline Monohydrate for the last 4 days or so. I don't really see any improvement. Also, over the last 5 years or so, I have had a crazy battle with a pretty intense jock itch. I am pretty confident I don't have an STD either as I have not been sexually active for almost 2 years, partially due to the adverse effects of Diabetes. I still can have an erection, but it seems to be more difficult to become fully erect when I want to. I know there may be several issues going on with me with everything I just described. But I am trying to find out why I have this intense pain in my testicle area. In addition to an increase pain in my testicles when leaning forward while sitting, sometimes it feels like something has minor movement from the top of my groin area to the lower left side of my abdomen. I don't think anything is truly moving, but I feel this awkwardness with the pain that is constantly there. Would you have any ideas what might be the issue? I have seen two doctors already, and really still am lost trying to find a solution to fix my problem.

I have read your query and understand your concerns. I see that you are suffering from intense pain in the left testicle and the surrounding areas since last 2 months. You are also type 1 diabetic.

In my opinion, it seems like an on-going inflammation in the testis and the epididymis area called as Epididymo-orchitis. Your description of pain suggests so. Your doctor is right in prescribing you Doxycycline for a month; please complete the course. It could be prostatitis as well. I would suggest you to continue the medicine even though you don't see any improvement as of now.

There is also a possibility of unnoticed inguinal hernia. Perhaps, the CT scan missed it.

At this stage, it would be wise to do a scrotal ultrasound to look at the underlying pathology, if any. Was there a history of trauma to your scrotum, anytime during these days? Do you notice any scrotal swelling?

It does not sound like STD to me either. How are your blood sugars now? Are they in good control? Let me know the latest values.

Again, the increasing pain in testicles when leaning forward, sitting makes me think of hernia as a possibility again. Why don't you consult a skilled surgeon and get examined for inguinal hernia again?

If you still do not find any good reasons, then you would have to repeat the CT scan.
