Brief Answer:
Consider DVT as one of the causes.

Detailed Answer:
Hi.
Thanks for your query.
To recapitulate: Male/81 - pain and swelling in calf - from knee to calf - 8 to 10 months - varicose veins on inside but the brownish discoloration on the outside - 2 months - pain is like toothache and hurts when walking or standing but not in sitting position - Doctor thought this to be a rash - gave cortisone shot and a script for antibiotic...

As per the history you have provided I would think of he following causative factor/s:
The swollen and painful left calf with the discoloration is indicative of a deep vein thrombosis and varicose veins.
I would advise you the following in such a situation:
- Ultrasonography and color doppler examination of both the lower limbs for comparing. This will give us the most proper diagnosis and a plan for treatment as per the findings.
I hope this answer helps you, please feel free to ask for further relevant queries if you feel that there is a gap of communication.
