Question: Hi
My partner has a mechanical heart valve and on Warfarin. He has an ulcerated foot - he attended 2 GPS - the 1st put him on penicillin antibiotics and cream - after one week the wound opened and the blood was shooting all over the place - it was stopped - he went back to same GP who told him - Carry on with the antibiotic and cream - his pulse was taken which was all over the place and blood pressure was high - nothing happened after the 2nd week with prescribed drugs - he saw another GP on Friday who told him this needed to be seen to - do not take any more penicillan - he did not understand why he would be prescribed this. Carry on with Paracetamol and get wound that looks like a small crater to the inside of the right foot near heel - He cannot Sleep - Shooting Pains -Redness and Swelling - What should we do !!!!

Sorry - missed this - Carry on with Paracetamol and get wound that looks like a small crater to the inside of the right foot near heel dressed weekly.

He also has varicous veins and is 40% disabled by the Armed Forces to a broken ankle sustained in Belize during a parachute jump in the 80s !

Brief Answer:
Consult angiologist immediately or send to ER

Detailed Answer:
Hi,

I am sorry for the health situation your partner is going through.

You are saying that:
- he has an opened wound on his foot that is bleeding and not healing
- penicillin did nothing
- he has also varicose veins
- and high blood pressure

To my opinion, you should consult an angiologist (specialist for blood vessels including varicose veins) as soon as possible. You should send him to ER if the wound is bleeding to get immediate help there.

The wound is because his foot is not being normally supplied with blood due to varicose veins.

Diabetes is to rule out as the wound is not healing. Fasting blood sugar, HBA1C levels are advised to check.

Meanwhile, monitor his blood pressure and report to the doctors in ER or to angiologist as antihypertensive medications should be started.

Wish fast recovery!
