Suggest Treatment For Obesity In A 4 Year Old Child

Question: Hi Doctor,
I hope you are doing good !!.
I am XXXX XXXXXXX and I am concerned about the health of my daughter (4 year old) XXXXXXX XXXXXXX
Brief history about her from birth:
Medical History:
1. She is fed on formula milk from day 1.
2. She has a problem of GER and we came to know that when she was 3 months old.
She was given "Histac" - 2 ml , twice a day for 4 months and domstall for few days.
3. She has an allergy , due to which she suffer cold and cough frequently. Doctor advised that it will continue till 15 years normally.
Eating habits:
1. She does not like outside food like pizza or any other fast food.
2. She like khichdi and daliya most and we give her khichdi or daliya once a day , normally in afternoon.
3. She ate 1/2 "parantha" in the morning (6.30 AM) when she leave for school and 2 paranthas in school at 11AM. In school she eats fruits @12.30 PM second break.
4. Evening we give her some milk and rusk/bread.
5. 7 PM , we give her dinner normally 2 chapati with vegetables. and she sleeps around 8 PM.

She plays a lot at home , but no speciall exercise or dance session.

Doctor we are conerned for her weight and stomach problems. She weighed around 23 KG at this age and have big stomach.
She has no complaints of stomach pain , but she go to loo for 3-4 a day.
Some times she felt very hungry and eat 3 chapati also and sometimes not even one.
She likes ice cream also and she ate it once in two days normally.

She has urinary infection almost daily , we need to clean her properly and apply some

I need your expert advise in this, I think she has abdominal issue which we are not able to check. Which medicine should we start and should we change diet also ?

Your kid is definitely overweight. There could be two reasons for this.
1. Endogenous obesity - due to genetic and family predisposition.
2. Exogenous obesity - due to lack of exercise and over eating.
I have a few questions for you -
1. What is her height now?
2. What was her birth weight?
3. From when did she start gaining weight like this (from what age)?
4. How are her daily activities and exercising pattern?
5. Are there any complaints from school about her academic performance?
6. Is there any family history of obesity?
7. Does she eat a lot of snacks?
8. How many hours does she watch TV per day?

1. What is her height now? -- height not known, but she is fine as per general physician we visited last time (2 months back). (Attach is the photo of kid , the middle one in pink top)
2. What was her birth weight? -- 2.4 KG
3. From when did she start gaining weight like this (from what age)? -- from last one year
4. How are her daily activities and exercising pattern? -- No exercise , She plays at home and we have floor wise home , so much rounds of stairs.
5. Are there any complaints from school about her academic performance? -- No
6. Is there any family history of obesity? -- No
7. Does she eat a lot of snacks? -- No .. only in evening that too milk and some rusks/breads
8. How many hours does she watch TV per day? -- 0 hours (but she watch you tube for 1 hour aorund daily)

Hi... I thank you for answering my questions with patience.

I have calculated her weight you have provided 23kg for a 4 year old) in comparison with her age. It is more and is falling at the 99th percentile.

But I have seen her image and I am very much convinced that she is a well kid.

I need more information please -

1. What is the exact date of birth?

2. What is her height now?

(please measure it by making her stand to the wall with back of feet and back and head touching the wall and mart the height by placing a card board on her head and then measure it with a scale or inch tape and then send it to me)

3. Any body else in the family is having rapid growth like this?

4. What are the ages of the kids standing next to her in the photo and are they relatives of you?

5. Is there any history of consanguinity (I mean were and your wife blood related before marriage)?

6. How was your growth and your wife's growth at this age (when you both were kids)?
