Suggest Treatment For Nerve Pain In The Groin Area

default
Posted on Mon, 20 Oct 2014
Question:

Gender: Female

Age: 40

Visited a Doctor: Yes

Medications Tried: I had severe left paraspinal spasms and pain at my left SI joint initially. Then that pain went away leaving me with nerve pain, sciatic in nature, but also had burning around to my groin and front thigh. They started with tramadol only increasing it finally from 1 tab every 8 hours to 2 tabs every 8 hours, then 2 tabs every 6 hours and added valium since I don't react well to muscle relaxants. Finally, after 6 weeks of this, and after having a small argument with my PCP to possibly try an extended pain reliever for a week 2x/day with the valium, (which he did not), he gave me a prescription for a short course of Percocet. I'm seeing him tomorrow. MRI should be back as well, but still in pain

I have ahd 11 left knee surgeries in the past and finally had a total knee replacement. I get occasional pain still and I take tramadol once in a while. my concern is that after I had to change my pcp and hospital, the doctors reading my file have diagnosed me with "chronic" knee pain. Now when I go in for any other type of pain related problem I think they think I'm drug seeking and I rarely get prescribed medication for pain trelief. Now I get severe anxiety any time the topic even arises. I begin to panic. what do I do? I can't seem to get the right care and sometimes I feel as though Jesus, what do I do , hit the streets to get treated?

I have discussed my feelings with the dr. but he just says not to worry about it, however, I have felt like ive been a nuisance and feel so embarrassed by their reactions. They did make me feel like Im some sort of drug abuser. That was a horrible feeling and my trust and faith in my dr is now fading. I'm scared to be honest.

Dear Madam,

Thank you for posting your query on HCM.

Well looking into your problem I would like to tell you that You need not feel anxious about all that . Pain is a perception and no doctor can measure it so we always have to go by the word of the patient . Its just the matter that what medicine should be best suited for you .

Since you say sciatica like pain , pain modulators like SSRI are best suited for you . Since they are not addicting your doctor would also not mind that . You can discuss this with your doctor . opioid based pain relieving medicines have addiction properties so doctors are more conservative with their use .

Rest pregabalin or gabapentin can also be used as pain modulators .

Take care.

But I'm taking Naprosyn 2-3x/day and Neurontin as well. Burning sensations has gone away but the deep pain has not. SSRI's lower seizure thresholds. I can not take them safely with another drug that can lower seizure thresholds (which is like everyone of them, but esp certain antibiotics and tramadol). My PCP will not prescribe SSRIs for that reason. I take tramadol sparingly. Im not sure what pain management specialist does? he mentioned an apt with them. what do u think?

Detailed Answer:
Dear Madam,

Thank you for reverting back.

You can use tramadol sparingly as you are using there is no harm . Pain management specialist basically titrates the doses of painkillers and selects which one are the best suited for you . You can discuss regarding SSRIs with your treating doctor , every case needs to be individualised.

Going to pain management clinic is a wonderful idea if its feasible for you and you must see a pain management specialist for the same.

Take care.
