Suggest Treatment For Nausea, High Bilirubin Levels, Vomiting And Stomach Discomfort

Question: my 25 year old son has had chronic stomach problems since grade 3/4. gotten worse since highschool.
his main complaint is nausea and the only thing that came up negative in all the tests he has had is
high billirubin. he also has vomitting and low apetite, stomach pain and discomfort, no constipation,
weight loss, and more...some rashes...had a bunch of tests done ...had an mri last summer abdominal..
my question is ...the family physician i believe will ok a hida scan gallbladder and that took some convincing..going this afternoon for him to sign the requisition form...i want him to sign for two tests
but he only knows about the one so far...at first we both wanted the hida scan but i thought it showed
liver and gallbladder...but this one is only for gallbladder only...so i asked my son ..i don't know what
to do...not much hida scan in our area...this form also has LIVER / SPLEEN SCAN option...my son wants
that...obviously i will do what my son wants...but not sure which do you think we should go for
considering he has had high billirubin in the past...the physicians diagnosed him with a physosomatic
disorder and suggested meditiation and lifestyle changes...and cyclic vomitting...but in my heart
i don't believe that...he doesn't fit the cyclic vomitting profile...
what test do you think we should go for...i will try to get both ...or we live in brampton XXXXXXX
do you know a hida scan place that does liver and gallbladder and spleen? any thoughts...thank you

Thanks for your query. I understand your concerns.

If he has got a high bilirubin then first thing we need to know that is it direct or indirect bilirubin. Because the causes and investigations are totally different. As a hida scan is being planned, I assume it is direct bilirubin.

In that case hida scan should be sufficient as MRI abdomen has already been done. Hida can't show spleen. If you want a nuclear scan of spleen you should look for spleen SPECT. In your Brampton nuclear services can do that in all probability. But I don't see the necessity of it at this point.

Also for recurrent vomiting I recommend having an upper GI endoscopy. Did he have that as well?
