Suggest Treatment For Muscle Injury In Pelvis

Question: Hello Doctor, Day before yesterday I had an accident..I was hit by an auto. I got some minor injuries on my forehead, knee and thigh which I feel not very serious. But the handle of the auto has hit my left pelvis. There was not much bleeding but the portion is painful though not too much. I am using an ointment and consulted with my Family Physician who has prescribed Cefpodoxime (Ycepo-200) and Hifenac-P, twice daily for five days. I am afraid if there is any internal damage or may arise in future. I can walk, sit and stand. It is not paining much but a mild pain is there. Please give me your valuable suggestion.

Please find my observations below.

1. It is possible that you have got a soft tissue or muscle injury in the pelvis. This causes mild pain which gets better in a week. The medicines prescribed are a pain reliever and antibiotic which are good ones.

2. Since you are having a doubt about internal injury, I suggest you get an ultrasound scan done to know if there is any injury. Many patients like you come to me with similar complaints and most of them do not have any injury. Few have slight swelling of muscles. Since there is no severe pain therefore it is unlikely to be a bone related problem and therefore I do not feel a X ray is required.

3. Please watch for any problems like passing blood in stool, sudden pain which is spreading to rest of abdomen and any problems in passing stools. If you have such symptoms then please consult your doctor again.

Thank you Doctor. There is no passing blood or pain in stool or urine, it's normal. But the pelvis portion and its surrounding is slight reddish like Blood clotting and a bit painful if I press with finger. If I stand up from sitting, I can feel a mild pain like stretching over there.

1. If stools and urine is normal then no need to worry. The discoloration is probably from minor bleeding under the skin which will get reabsorbed in a week. If the redness or the swelling increases then you can get an ultrasound scan done.

Thank you very much Doctor. My last question is do I need to take a Tetanus vaccine? I had Functional endoscopic sinus surgery two years back in 2013. At that time I was probably vaccined, so my Family Physician suggested me that I don't need to take one vaccine now. What is your suggestion?

Tetanus toxoid is given once in 10 years as booster

Thanks for writing back with an update.

1. Tetanus toxoid vaccine is given every 10 years as a booster dose. Tetanus toxoid injection is better taken within 24 hours of injury.

2. Since you had one 2 years back therefore it is not essential.

3. However it is safe injection and you can still take it if there is significant injury and contamination with dust. If you have taken the vaccination earlier in 2013 then this will act like a booster and next booster is to be given after 10 years.
