Suggest Treatment For Mild HTN, Chest Pain, Light-headed Feeling And Shoulder Pain

Question: I am a 33 year old female with significant family history of heart disease, to include losing my dad to MI at age 52. My MGF has had 2 heart attacks, a quad and triple bypass surgeries. I was diagnosed with LBBB at age 25 but released with no f/u required. 7 months ago I was diagnosed with mild HTN and started on 50mg Metoprolol BID. Since then my BP has been controlled but I've been having chest pain (from under my sternum to slightly (my) left of sternum) off and on for several months as well.

Yesterday I started having chest pain around 9am while sitting at my desk, and it continued all day at 3/10 pain, at times going up to 5/10. I took some indigestion medicine thinking maybe it was gas but it didn't help. When I woke up this morning I felt fine but by the time I got to work was having the pain again. As the morning progressed I began getting light-headed, having pain in my left shoulder and my left arm was going numb. At this point I was very concerned and went to my doctor.

The EKG result was "sinus tachycardia, first degree AV block and intra-atrial conduction delay" and the details are as follows:
P/PR - 140/236 ms
QRS - 92 ms
QT/QTc - 336/452 ms
P/QRS/T - 55/63/17 deg

A few more details: I am also being treated for fibromyalgia, PCOS, hashimoto's thyroiditis, HTN and Hyperlipidemia. Also have history of migraines and severe headaches.

You are experiencing a new onset chest pain. It's a good thing that nothing was found worrisome on your ECG. However other examinations are needed to exclude any cardiac problems.

If you were my patient I would recommend TESTS to do:
- Cardiac ECHO
- Stress test
- Holter monitoring
- Abdominal ultrasound
- Chest X ray
This will help to find out the exact nature of your pain.
