Suggest Treatment For Lump Inside The Uvula

Question: Hi Kumaresh Sir,
How are you? I came to you when I was in XXXXXXX in 2010. Later I moved to US and now I am having certain problems and want to take your opinion.

-On May 2nd 2015 after returning from a small picnic, I notices a small lump under jaw right side. It is very small I guess lymph node. It will be a bit in the morning and will condense to very small 2mm by afternoon.

- After 3 weeks I went to vacation to different state and when I came back I stuck with cold for couple of days. I usually get cold when I go for vacation or change place. I have used tylenol and it has subsided. At the time when I got cold, I have observed another lymph node very small on right side of neck. It wont appear to be there, but when we tough and rub hard we can feel it. It is wrapped with a muscle and I can say that it is just about 2 rice grains.

- Mean while I was worried and first went to local doctor and used Amox for 1 week course. At that time the right side node was a bit big like pea but after using it the size got reduced but it wont go away. I feel like it little big morning times but later in the day it becomes very small.

I was worried and went to ENT specialist here, he has seen both the nodes and examined E,N,T and said nothing to worry and he said he has not found anything abnormal.

- I was not convinced and later went to a surgeon and she said that she would do an endoscope of neck and she did. Results are pending.

-While this is happening morning out of curiosity I have checked all my mouth and found that there is a little mass inside uvula . When I turn uvula back the mass appears. It appears that the mass is from inside.

Just want to know if you connect the dots with whats happening to me and suggest is it something major I have to worry about.

I have full trust on you as I was your previous patient.

I have attached some images for reference. I want to come to XXXXXXX or take quick action if this is something major. I was an occasional smoker.

I have gone through the query and examined the images thoroughly.

My examination findings are:
1. Congestion over the tonsillar pillars, soft palate and uvula. Also, an isolated swelling is noticed on the uvula. The swelling could either be a blocked minor salivary gland or a cyst (fluid filled sac).
2. Congested and granular pharyngeal wall with post nasal drip.

In a nut shell, there is infection still persisting in the throat (tonsillo-pharyngitis) which would require broad spectrum antibiotics. The enlargement of the lymph nodes could be due to tonsillo-pharyngitis.

My suggestions to you are:
1. Drink plenty of warm water.
2. Try using dilute povidone iodine throat gargles.
3. If you are having gastritis, try taking medicines like proton pump inhibitors.
4. Also, try taking a course of broad spectrum antibiotics.
5. Avoid chilled food and beverages, fizzy drinks, coffee and smoking.

I am waiting for the endoscope results of neck. Does this tell if it is a cyst or salaivary gland?

The present images shows edematous uvula. I don't see any cyst or mucocele (blocked minor salivary gland).

Do not worry! This seems to be getting better. Please follow those measures which I mentioned in my previous response, you will definitely get better. Drink plenty of warm water, to keep yourself hydrated and also to reduce edema over uvula.

Today I went to Dr XXXXXXX L. Ziffra, MD in XXXXXXX He has examined me using some pointed torch in nose, throat. He said he has also examined the back side of my uvula. He said he can feel the lymph nodes but they are very small each one side of neck which need not to be worried unless they become larger than 2 cms.

He said he has seen the uvula back side also and said the uvula looks normal and he assured that what he can see are mucous glands.

When I said but I have not seen them there previously, he said I might not have seen back side of the uvula.

He said overall my examination was normal and said there is no need for any X-Ray or any further examination.


When I repeatedly asked about the mass on uvula, he reassuringly said that those are mucous glands.

Just wondering if there is any chance in his examination to miss something major?

Welcome back

Trust me, there are no major issues in you. The swelling over the uvula is just a part of inflammatory changes and it reduces once the foci of the infection reduces.

Similarly, the granulations over the posterior pharyngeal wall appear during the episodes of infection, mouth breathing, allergy and acid reflux disease. Do not worry! Once the inciting agent is taken care of, the inflammation will subside automatically.

I got by Ultrasound Neck Results.

The doctor says that there is a small cyst near the parotid gland and she thinks that it is benign and wants to repeat the ultrasound in next 3 months to see if it gets bigger.

She also tells that there is one lymph node enlarged on right side.

I am assuming that the lymph node is enlarged because of the cyst.

Please connect all the dots mentioned and let me know is it something minor problem or major.

I do understand your apprehension regarding this new finding. My interpretations are; it could be a harmless mucus filled cyst, which is not infected. The lymph node, if noticed in front of the gland could be secondary to infection in the parotid gland, whereas, the one noted below the mandible or cervical region may not be related to the former.

As most of the swellings inside the parotid gland are benign, there is no role for any aggressive intervention. As your doctor has recommended, we can wait and watch for any enlargement in the cyst.

Finally, both the parotid cyst and uvular swelling are not inter-related.
