Brief Answer:
Tramadol , pregabalin have longer duration of action than that of narco in addition to steroid injections are effective with most cases else a neurosurgeon point of view is recommended for proper evaluation and management .

Detailed Answer:
Hi

I had gone through your question and understand your concerns ..

From your given history of sciatica , disc herniation and low back pain I recommend avoid heavy lifting , vigorous movements , bad sleeping and sitting postures , overweight .
Physiotherapy , spinal straightening exercise , hot massages with topical gels and even yoga are helpful .

You may ask your physician to use tramadol 200 mg tab twice daily instead of narco as it have longer duration of action than narco , pregabalin 400 mg tab once daily , muscle relaxants , B12 vitamin supplementation , steroid injections will be effective for such cases .
If pain persists for more than 6 month i think you may need a neurosugeon minor endoscopic intervention to release your neurovascular compression causing that kind of chronic neuropathic pain.

Wish you good health,
Any further clarifications feel free to ask.
